专科护士主导下的交互式平台在腹膜透析患者健康管理中应用研究

Research on the Application of Interactive Platform Led by Specialized Nurses in the Health Management of Peritoneal Dialysis Patients

目的:探讨专科护士主导下的互联网交互式平台在腹膜透析患者健康管理中应用的有效性。方法:采用便利抽样法抽取2017年5月-2018年5月笔者所在科室收治的持续行腹膜透析的患者236例,根据知情同意原则,入选观察组118例,对照组118例,对照组采用传统随访模式,观察组采用专科护士主导的交互式平台进行随访。专科护士随访1年,护士每3个月收集一次临床数据和参数。结果:观察组的患者在基础知识掌握情况、自我管理行为能力、患者生活质量及满意度方面与对照组比较差异均有统计学意义(P<0.001)。结论:专科护士主导下的交互式平台对腹膜透析患者完善自我认知,增强自我管理信心,提高自身健康管理行为是有效的,这说明可以尝试在慢性疾病患者中建立一套具有推广和实际操作意义的专科患者交互式平台随访模式系统,为慢病管理提供一个可借鉴、可复制的模式。

Objective:To explore the effectiveness of internet interactive platform led by specialized nurses in the health management of peritoneal dialysis patients.Method:A total of 236 continuous peritoneal dialysis patients admitted in my department from May 2017 to May 2018 were selected by convenient sampling method.According to the principle of informed consent,118 patients in the observation group and 118 patients in the control group were selected.The traditional follow-up mode was adopted in the control group,and the interactive platform led by specialized nurses was adopted in the observation group for followup.The nurses were followed up for one year and collected clinical data and parameters every three months.Result:Compared with the control group,the patients in the observation group had significant differences in basic knowledge,self-management behavior,renal quality of life and satisfaction(P<0.001).Conclusion:The interactive platform led by specialized nurses is effective to improve the self-awareness,self-management confidence and self-health management behavior of peritoneal dialysis patients,which indicates that we can try to establish a set of interactive platform follow-up mode system for specialized patients with chronic diseases,which has the significance of promotion and practical operation,and provide a reference and a recovery mode of system for chronic disease management.
