奥马哈系统对肿瘤患者PICC生理领域结局护理效果的影响研究

The effect of continuing nursing based on Omaha system on PICC outcomes in tumor patients

目的分析奥马哈系统对肿瘤患者经外周中心静脉置管(PICC)生理领域护理结局的影响。方法选取2017年2月-12月某三级公立医院置入PICC行化疗的肿瘤患者,实施常规导管护理46例患者为对照组,运用奥马哈系统护理50例患者为试验组,分别在置入PICC当天及前3次化疗结束时(共4次)对患者生理领域护理问题的行为、认知、状况进行评价,比较2组患者护理结局的改善情况。结果2组患者对认知、疼痛、皮肤、神经肌肉、感染、循环生理领域问题的认知、行为、状况均随着评价时间变化维度得分呈增长趋势,而且试验组平均维度得分均高于对照组,时间、组间效应之间差别均有统计学意义(P<0.05)。结论将奥马哈系统运用到肿瘤患者PICC护理中能够提升患者对生理领域问题的认知、行为,改善护理结局。

Objective Purpose To evaluate the nursing outcome in physiological field based on the Omaha system on PICC care applied to tumor patients.Methods Tumor patients undergoing PICC chemotherapy were selected from February to December 2017.There are 46 patients with PICC catheters were selected as the control group with the routine nursing care and 50 patients were selected as experimental group and carried out nursing based on the Omaha system.The cognition,behavior,and status of patients’nursing problems were evaluated on the day of PICC implantation and at the end of the first three chemotherapy sessions(4 times)to compare the improvement of nursing outcome between the two groups.Results The cognition,behavior and status of cognition,pain,skin,neuro-muscle,infection and circulation physiology in these two groups increased with the change of evaluation time,and the average score of the experimental group was higher than that of the control group,and differences in time and effect between these two groups were statistically significant(P<0.05).Conclusion The application of Omaha system to PICC nursing of tumor patients can improve the cognition and behavior of patients about physiological problems and can improve the nursing outcome.
