耳穴贴压法潜伏期分娩镇痛对产妇疼痛程度及炎性细胞因子表达的影响

Effects of auricular point press therapy for labor analgesia in the latent period on pain degree and expression of inflammatory cytokines in parturients

目的评价耳穴贴压法潜伏期分娩镇痛对产妇疼痛程度及炎性细胞因子表达的影响。方法选取我院2018年1月~2019年3月阴道分娩初产妇100例,采用随机数字表法分为观察组和对照组,观察组产妇潜伏期利用耳穴贴压法镇痛,对照组产妇采用传统分娩方式,观察产妇不同时期炎性因子表达水平,比较两组新生儿Apgar评分、第一产程时间、镇痛满意率及VAS评分。结果两组产妇在不同时期的白介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)的蛋白活性差异有统计学意义(P<0.05);在T2~6时期,观察组产妇以上指标明显低于对照组,差异有统计学意义(P<0.05);观察组新生儿在1、5min的Apgar评分与对照组比较差异无统计学意义(P>0.05);观察组第一产程时间、镇痛满意率均显著优于对照组,差异有统计学意义(P<0.05);观察组产妇潜伏期VAS评分显著低于对照组(P<0.05),但活跃期评分两组比较差异无统计学意义(P>0.05)。结论对潜伏期分娩镇痛采用耳穴贴压法可以明显的降低炎性细胞因子的活性,缩短产妇第一产程的时间,降低产妇的疼痛程度,同时还可以增加产妇对镇痛的满意率。

Objective To evaluate the effects of auricular point press therapy for labor analgesia in the latent period on pain degree and expression of inflammatory cytokines in parturients.Methods 100 primiparae with vaginal delivery at our hospital from January 2018 to March 2019 were selected,and were divided into the observation group and the control group by using therandom number table methods.Auricular point press therapy was applied in the latent period for analgesia to parturients in the observation group,while traditional delivery mode was applied to parturients in the control group.The expression levels of inflammatory factors in different periods of parturients were observed.The neonatal Apgar score,duration of first stage of labor,satisfaction to analgesia,and VAS score were compared between the two groups.Results The differences in protein activity of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)at different periods between the two groups of parturientswerestatistically significant(P<0.05);in the T2-6 period,the above indexes of parturients in the observation group were significantly lower than those in the control group,and the difference was statistically significant(P<0.05);the difference in neonatal 1min and 5min Apgar scores between the observation group and the control group was not statistically significant(P<0.05);the duration of first stage of labor and satisfaction to analgesia in the observation group were significantly better than those in the control group,and the difference was statistically significant(P<0.05);the VAS score in the latent period of parturients in the observation group was significantly lower than that in the control group(P<0.05);but the difference in score in active phase between the two groups was not statistically significant(P>0.05).Conclusion The application of auricular point press therapy for labor analgesia in the latent period can significantly reduce the activity of inflammatory cytokines,shorten the first stage of labor,reduce the pain degree of parturients,and enhance the satisfaction to analgesia of parturients.
