摘要
目的研究儿童过敏性紫癜不同时期血清D-二聚体水平、尿β2微球蛋白水平,探讨其临床意义。方法选取儿科住院部2017年1月~2018年12月收治的50例过敏性紫癜患儿作为实验组,30例健康儿童为健康组,检测不同时期血清D-二聚体、尿β2微球蛋白水平;实验组又分为单纯型组和混合型组,比较两组在相同病程中血清D-二聚体、尿β2微球蛋白水平;并分析血清D-二聚体水平与病程的相关性。结果实验组患儿入院1 d(未治疗前)、病程1个月、病程6个月时的血清D-二聚体、尿β2微球蛋白水平高于健康组,差异有统计学意义(P<0.05);混合型组患儿入院1 d、病程1个月、病程6个月时的血清D-二聚体、尿β2微球蛋白水平高于单纯型组,差异有统计学意义(P<0.05);病程1个月后混合型组D-二聚体阳性率明显高于单纯型组,差异有统计学意义(P<0.05)。结论过敏性紫癜患儿病程早期的血清D-二聚体、尿β2微球蛋白水平不能准确反映病情,但病程超过1个月后,血清D-二聚体、尿β2微球蛋白水平增高提示病情迁延,需动态监测。
Objective To study the levels of serum D-dimer and urineβ2 microglobulin in children with allergic purpura at different stages,and to explore its clinical significance.Methods Fifty children with allergic purpura admitted to the Pediatric Inpatient Department from January 2017 to December 2018 were enrolled as experimental group,30 healthy children were as healthy group.The serum D-dimer,urineβ2 microglobulin levels at different stages were detected.The experimental group were further divided into simple group and mixed group.The serum D-dimer and urineβ2 microglobulin levels were compared in the same course of the two groups.And the relevance of serum D-dimer levels and the course of the disease were analyzed.Results The serum D-dimer and urineβ2 microglobulin levels were higher in the experimental group than those in the healthy group at one day(before treatment),one month,6 months after admission,and the difference was statistically significant(P<0.05).The serum D-dimer and urineβ2 microglobulin levels in the mixed group were higher than those in the simple group at one day(before treatment),one month,6 months after admission(P<0.05).After the disease course was more than 1 month,the positive rate of D-dimer in the mixed group was significantly higher than that in the simple group,and the difference was significant(P<0.05).Conclusion The levels of serum D-dimer and urineβ2 microglobulin in the early stage of children with allergic purpura can not accurately reflect the disease.But after the disease course exceeds one month,the increase of serum D-dimer and urineβ2 microglobulin levels suggest that the disease is prolonged and needs dynamic monitoring.
