支气管哮喘患者FeNO测定与肺功能及外周血嗜酸性粒细胞的相关性研究 被引量:4

摘要 目的探讨支气管哮喘患者FeNO测定与肺功能及外周血嗜酸性粒细胞的相关性。方法选择2017年1月~2019年1月就诊于我院门诊或住院的哮喘急性发作期患者80例,按照哮喘控制检测(Asthma control detection,ACT)评分分为轻度组、中度组、重度组。同时,选取30位健康体检者作为健康对照组,哮喘组患者用药前完成哮喘控制检测。之后给予4周信必可都保治疗。观察三组患者治疗前后FeNo水平、外周血嗜酸性粒细胞比例、第1秒用力呼气量占预计值百分比,分析FeNo水平与其他指标的相关性。结果三组患者在治疗前,FeNo水平、外周血嗜酸性粒细胞比例均高于对照组,第1秒用力呼气量占预计值百分比低于对照组,差异显著(P<0.05);三组患者FeNo水平经治疗后,均得到明显改善,与治疗前对比,差异显著(P<0.05);FeNo水平与第1秒用力呼气量占预计值百分比无明显相关性(P>0.05),与外周血嗜酸性粒细胞比例表现呈正相关(P<0.05)。结论支气管哮喘患者FeNO测定与外周血嗜酸性粒细胞有明显的相关性,临床可根据其水平变化,辅助临床诊治。 Objective To study the correlation of FeNO measurement with pulmonary function and peripheral blood eosinophils in patients with bronchial asthma.Methods Eighty patients who were admitted to the outpatient department or hospitalized in our hospital with acute asthma attack from January 2017 to January 2019 were selected as subjects.According to the score of asthma control detection(ACT),they were divided into the mild group,moderate group and severe group.Meanwhile,30 healthy subjects were selected as the healthy control group.Asthma control detection was performed on the patients in the asthma group before medication.Afterwards,symbicort turbuhaler was given to the patients for four weeks.The FeNo levels,proportion of peripheral blood eosinophils,the percentage of forced expiratory volume in the first second in the estimated value were observed before and after treatment in the three groups,and analyze the Correlation between FeNo Level and other indicators.Results Before treatment,the leves of FeNo and the proportion of peripheral blood eosinophils in the three asthma groups was higher than that in the control group,and the percentage of forced expiratory volume in the estimated value in the first second in the three asthma groups was lower than that in the control group,both with significant differences(P<0.05).After treatment,FeNo levels were significantly improved in all three asthma groups,with significant difference compared with those before treatment(P<0.05).The leves of FeNo was not significantly correlated with the percentage of forced expiratory volume in the first second in the estimated value(P>0.05).The leves of FeNo was positively correlated with the proportion of peripheral blood eosinophils(P<0.05).Conclusion FeNO measurement in patients with bronchial asthma has obvious correlation with peripheral blood eosinophils.It can be used as an auxiliary in clinical diagnosis and treatment according to its level.
