单孔腹腔镜与多孔腹腔镜卵巢囊肿剔除术的临床对照研究

Chinical comparative study of single-hole laparoscopic and multiport laparoscopic ovarian cystectomy

目的分析单孔腹腔镜、多孔腹腔镜卵巢囊肿剔除术、单孔腹腔镜基础上应用罗哌卡因三种方式的临床效果。方法选择2017年9月~2019年4月来自本院的妇科卵巢良性患者作为本次研究对象,根据随机分组方式对其进行分组,将300例患者进行分组,主要包括三组,其中单孔组实施单孔腹腔镜手术操作;三孔组实施多孔腹腔镜手术操作;联合组在单孔腹腔镜手术操作基础上术毕于脐部应用罗哌卡因,将三组手术指标、术后疼痛评分进行对比。结果联合组患者手术时间(39.21±1.52)min长于三孔组,术中出血量(22.12±1.02)mL、术后排气时间优于三孔组(P<0.05);联合组术后1 d(2.12±0.32)分、术后3 d(1.21±0.12)分、术后3个月(1.01±0.05)分、术后半年疼痛评分(0.25±0.01)分低于单孔组(P<0.05);单孔组术后1 d(3.01±0.15)分、术后3 d(1.95±0.21)分、术后3个月(1.56±0.17)分、术后半年疼痛评分(0.47±0.29)分低于三孔组(P<0.05)。结论单孔腹腔镜与多孔腹腔镜卵巢囊肿剔除术的临床对照发现,单孔腹腔镜基础上应用罗哌卡因更具有优势,其能够减轻术后疼痛感。

Objective To analyze the clinical effect of ingle-hole laparoscope, multiport laparoscopic ovarian cystectomy, ropivacaine based on single-hole laparoscope. Methods Benign gynecological ovarian patients in our hospital from September 2017 to April 2019 were selected as the subjects of this study. 300 patients were grouped according to random grouping, mainly including three groups. Among them, single-hole group performed single-hole laparoscopic operation;three-hole group performed multiport laparoscopic operation;combined group applied ropivacaine to the umbilicus after surgery based on single-hole laparoscopic operation. And surgical indicators and postoperative pain scores among the three groups were compared. Results The operation time of the combined group was(39.21±1.52) minutes,longer than that of the three-hole group. The intraoperative blood loss(22.12±1.02) mL and the postoperative exhaust time of the combined group was better than that of the three-hole group(P<0.05). The pain score at 1 day after surgery(2.12±0.32) points, at 3 days after surgery(1.21±0.12) points, at 3 months after surgery(1.01±0.05) points, and the pain score at half a year after surgery(0.25±0.01)points in the combined group was lower than that of the single-hole group(P<0.05). The pain score at 1 day after surgery(3.01±0.15) points, at 3 days after surgery(1.95±0.21) points, at 3 months after surgery(1.56±0.17) points, and the pain score at half a year after surgery(0.47 ±0.29) points of the single-hole group was lower than that of the three-hole group(P<0.05). Conclusion The clinical comparison between single-hole laparoscopy and multiport laparoscopy for ovarian cystectomy found that the application of ropivacaine based on singlehole laparoscopy is more advantageous, and it can reduce postoperative pain.
