彩超引导下对烧伤患者行改良塞丁格技术PICC置管的应用

摘要 [目的]探讨彩超引导下行改良塞丁格技术PICC置管对烧伤患者的运用效果。[方法]选取我院2018年6月至2018年12月42例烧伤患者,行彩超引导下的改良塞丁格技术PICC置管术。观察2组一次穿刺成功率、一次置管成功率及导管留置时间、并发症发生率、患者满意度。[结果]研究组一次穿刺成功率、一次置管成功率为95.23%、90.47%,高于对照组66.66%、47.61%(χ^2=7.71、χ^2=9.024,P<0.05);导管留置时间为(112.48±15.26)d,长于对照组(93.67±11.28)d(t=6.962,P<0.05)。2组导管堵塞、静脉炎、穿刺点出血、穿刺点感染、导管脱出发生率比较差异均无统计学意义(P>0.05);而并发症总发生率研究组(9.52%)明显低于对照组(38.09%),差异有统计学意义(χ^2=4.725,P<0.05)。患者置管满意度研究组(100%)显著高于对照组(76.19%),差异有统计学意义(χ^2=5.675,P<0.05)。[结论]烧伤患者行彩超引导下的改良塞丁格技术PICC置管术能够显著提高置管成功率,降低并发症率,安全可靠,患者满意度高。 [Objective]To study the application effect of PICC catheterrization by improving piercing technology of Seldinger under guide of ultrasonic on burn patients. [Methods]Forty-two burn patients admitred in our hospital from June 2018 to December 2018 were enrolled in the study. The patients were divided into study group and control group, each with 21 patients. The patients in the study group underwent PICC catheterization under the guidance of color doppler ultrasound. The success rate of one-time puncture, the catheter indwelling time, the incidence of complications, and patient satisfaction were observed between the two groups. [Results]The success rate of one-time puncture and one-time catheter placement in the study group were 95.23% and 90.47%, which were higher than the control group by 66.66% and 47.61%(χ^2=7.71、9.024,P<0.05). The indwelling catheter time was(112.48±15.26) days and longer than the control group(93.67±11.28)d(t=6.962,P<0.05). There was no significant difference in the incidence of catheter blockage, phlebitis, puncture point bleeding, puncture point infection, and catheter prolapse between the two groups(P>0.05);the total incidence of complications in the study group(9.52%) was significantly lower than the control group( 9.52%), and the differences were statistical Significance( 38.09%). Patient satisfaction(100%) was significantly higher than the control group(76.19%), and the difference was statistically significant(χ^2=5.675,P<0.05). [Conclusion]The use of PICC catheterrization by Improving Piercing Technology of Seldinger under guide of ultrasonic in burn patients can significantly improve the success rate of catheterization, reduce complications, be safe and reliable, and have high patient satisfaction.
