ICU冠心病患者超声心动图与心肌酶的关系及意义

The relationship and significance between the echocardiography and myocardial enzymes of ICU patient with coronary heart disease

目的探讨ICU冠心病患者的超声心动图与心肌酶水平的关系和意义。方法选取我院2018年7月至2019年7月在ICU治疗的冠心病患者50例作为观察组;并选取同一时期来我院的健康体检者50例作为对照组。应用彩色多普勒超声诊断仪行超声心动图检查;并采集两组的空腹外周静脉血,分离血清,并采用酶速率法测定肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)和羟丁酸脱氢酶(HB-DH)水平。结果应用超声心动图检测冠心病的检出率为84.0%;观察组左室收缩末期容积(LVESV)和左室舒张末期容积(LVEDV)明显高于对照组,左室射血分数(LVEF)明显低于对照组,差异具有统计学意义(P<0.05)。观察组的CK、CK-MB和HB-DH水平明显高于对照组,差异具有统计学意义(P<0.05)。经Pearson检验,冠心病患者血清CK、CK-MB和HB-DH水平均与LVEF呈直线负相关性(r分别=-0.684、-0.658、-0.662,P均<0.001)。结论ICU冠心病患者超声心动图和心肌酶水平呈现出良好的线性关系,可能对心肌损伤程度具有一定的指示作用,对病情评估有重要价值。

Objective To explore the relationship between echocardiography and myocardial enzyme levels in ICU patients with coronary heart disease.Methods Fifty patients with coronary heart disease treated in ICU in our hospital from July 2018 to July 2019 were selected as the observation group.Fifty healthy patients who came to our hospital in the same period were selected as the control group.Echocardiography was performed on ICU patients with coronary heart disease using cardiac doppler ultrasound.To collect all the research object of the morning on an empty stomach peripheral venous blood,separation of serum,and enzyme rate method is used to determination the levels of creatine kinase(CK),creatine kinase isoenzyme(CK-MB)and hydroxybutyric acid dehydrogenase(HB-DH).Results The positive rate of 50 patients was 84%by echocardiography.The levels of LVESV and LVEDV in the observation group were significantly higher than the control group,while the LVEF in the observation group was lower than the control group,and the difference between the two groups was statistically significant(P<0.05).The levels of CK,CK-MB and HBDH in the observation group were significantly higher than those in the control group,and the difference between the two groups was statistically significant(P<0.05).Pearson test showed that the serum levels of CK,CK-MB and HBDH in coronary heart disease patients were negatively correlated with LVEF(r=-0.684,-0.658 and-0.662,respectively,P<0.001).Conclusion Echocardiography showed a good linear relationship with myocardial enzyme levels in ICU patients with coronary heart disease,which may be an indicator of the degree of myocardial injury and of great value for the evaluation of the disease.
