超声检查联合CA125、OPN检测在卵巢癌诊断中的应用

Applied value of ultrasound combined with CA125 and OPN in the diagnosis of ovarian cancer

目的探讨超声检查联合糖类抗原125(CA125)、骨桥蛋白(OPN)检测在卵巢癌诊断中的应用价值。方法选取2017年1月至2018年3月在我院治疗的卵巢肿瘤患者89例,其中良性肿瘤51例,恶性肿瘤38例,均采用彩色超声检查,同时检测血清CA125和OPN水平。结果超声检查诊断卵巢癌灵敏度、特异度、阳性预测值和阴性预测值分别为76.32%、92.16%、87.88%和83.93%;恶性肿瘤患者血清CA125和OPN分别为143.92(34.38,210.29)U/ml和167.28(41.29,200.82)ng/ml,明显高于良性肿瘤(P<0.05);血清CA125、OPN判断卵巢良恶性肿瘤的ROC曲线下面积分别为0.887和0.865,截断值分别为54.26 U/ml和67.58ng/ml,灵敏度分别为84.10%和81.60%,特异度分别为90.20%和89.88%;超声检查联合CA125和OPN检测诊断卵巢癌灵敏度、特异度、阳性预测值和阴性预测值分别为92.11%、82.35%、79.54%和93.33%。结论超声检查联合CA125、OPN检测在卵巢癌诊断中具有较好的应用价值,值得临床应用。

Objective To investigate the value of ultrasound combined with carbohydrate antigen 125(CA125)and osteopontin(OPN)in the diagnosis of ovarian cancer.Methods From January 2017 to March 2018,89 patients with ovarian tumors were treated in our hospital,including 51 benign tumors and 38 malignant tumors,were examined by color Doppler ultrasound and the levels of serum CA125 and OPN were measured simultaneously.Results The sensitivity,specificity,positive predictive value and negative predictive value were 76.32%,92.16%,87.88%and 83.93%respectively;The serum levels of CA125 and OPN in malignant tumors were 143.92(34.38,210.29)U/ml and 167.28(41.29,200.82)ng/ml,which were significantly higher than those in benign tumors(P<0.05);The ROC curves of serum CA125 and OPN for benign and malignant ovarian tumors were 0.887 and 0.865,P<0.05,the cut-off values were 54.26 U/ml and 67.58 ng/ml,the sensitivity were 84.10%and 81.60%,and the specificity were 90.20%and89.88%;The sensitivity,specificity,positive predictive value and negative predictive value of ultrasound combined with CA125 and OPN were 92.11%,82.35%,79.54%and 93.33%.Conclusion Ultrasound combined with CA125 and OPN has better value in the diagnosis of ovarian cancer,and is worthy of clinical application.
