基于随访的自我管理指导对脑膜瘤术后患者自护能力及生活质量的影响

Effect of Follow-up Self-managerment Guidance on Self-care Ability and Life Quality in Postoperative Meningioma Patients

[目的]探讨基于随访的自我管理指导对脑膜瘤术后患者自护能力及生活质量的影响.[方法]选择2016年8月至2018年8月于本院行手术治疗的102例脑膜瘤患者,根据随机数字表法将其分为观察组和对照组,各51例.对照组给予包括健康教育、心理疏导、康复锻炼、并发症预防等在内的脑膜瘤术后常规护理干预,观察组在对照组的基础上给予基于随访的自我管理指导模式干预.随访3个月,比较两组患者自护能力、并发症、生活质量等指标.[结果]随访3个月,两组患者主动肢体功能锻炼、被动肢体功能锻炼、日常活动自理、完全自理均优于干预前,且观察组优于对照组,差异有统计学意义(P<0.05).干预前,两组患者躯体功能等评分比较,差异无统计学意义(P>0.05);随访3个月,两组患者躯体功能、角色功能、情绪功能、社会功能评分高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05).观察组患者术后并发症发生率为7.84%(4/51),低于对照组的19.61%(10/51),差异有统计学意义(χ^2=4.32,P<0.05).[结论]基于随访的自我管理指导模式有助于促进脑膜瘤术后患者自护能力的养成,减少术后并发症的发生,提高患者生活质量.

[Objective]To explore the efect of fllow up self management guidance on self care ability and life quality in postoperative meningioma patients.[Methods]One hundred and two patients with meningioma surgery from August 2016 to August 1818 were enolled.The subjects were divided into treatment group and control group by using random number table method,51 cases in each group.The control group received routine nursing intervention for meningioma including health education,psychological counseling,rehabilitation exercise,and prevention of complications.The treatment group was combined with self-management guidance mode based on follow up.Three months follow-up was performed to compare the self protection ability,complications,quality of life and other indicators of the two groups.[Results]In the treatment group,the self protection ability index,active and passive physical function training,daily activity self care and complete self care were significanly higher than those of the control group(94.12%vs 70.59%,98.04%vs 78.43%,100.00%vs 70.59%,92.17% vs 72.55%)(χ^2=8.163,7.650,15.320,5.464,P<0.05,P<0.01);physical function,role function,emotional function and social function score were significantly higher than those of the control group(1=5.932.4.125,5.770,4.972,P<0.05);the incidence of complica tions such as pulmonary infection was 3.92%,which was significantly lower than that of the control group 45.10%(χ^2=40.267,P<0.01).[Conclusion]The self manage-ment guidance model based on follow up can help to promote the self-care ability of postoperative meningioma patients,reduce the incidence of postoperative complcations and improve the quality of life of patients.
