Correlation Between Thyroid Function and Severity of Coronary Heart Disease in Perimenopausal Women

摘要 目的探讨围绝经期女性甲状腺功能与冠心病及其严重程度的相关性。方法回顾性分析2018年6月至2019年6月于郑州大学第一附属医院心内科住院治疗、拟诊冠心病并行冠状动脉造影术的165例围绝经期女性,计算Gensini评分。排除低T3综合征患者后,根据冠脉造影结果分为非冠心病组(110例)和冠心病组(54例),比较两组一般资料、甲状腺功能、血脂、血肌酐的差异性。多因素logistic回归分析冠心病的危险因素,应用Spearman秩相关分析甲状腺功能与Gensini评分以及血脂的相关性。结果两组FT3、TG比较,差异有统计学意义(P<0.05)。多因素logistic回归分析示,FT3是冠心病的保护因素(OR=0.335,95%CI:0.166~0.677),TG是冠心病的危险因素(OR=1.545,95%CI:1.007~2.371);Gensini评分与FT3呈负相关(r=-0.488,P<0.05),甲状腺功能与血脂无相关性。结论在非低T3综合征的围绝经期女性中,FT3与冠心病及其严重程度呈负相关。FT3减低可能是影响冠心病发病的重要因素之一。在围绝经期冠心病患者中检测甲状腺功能具有十分重要的临床意义。 Objective To explore the correlation between thyroid function and the severity of coronary heart disease in perimenopausal women.Methods A retrospective analysis was performed on 165 perimenopausal women who were admitted to the department of cardiology of the First Affiliated Hospital of Zhengzhou University from June 2018 to June 2019 for treatment of coronary heart disease and coronary angiography, and Gensini scores were calculated. After excluding patients with low T3 syndrome, 110 patients were divided into the non-coronary artery disease group and 54 patients were divided into the coronary artery disease group according to the results of coronary angiography. Multivariate logistic regression was used to analyze the risk factors of coronary heart disease. Spearman rank correlation was used to analyze the correlation of thyroid function with Gensini score and blood lipid.Results There were statistically significant differences in FT3 and TG between the two groups(P<0.05). Multivariate logistic regression analysis showed that FT3 was a protective factor for CHD(OR=0.335, 95%CI: 0.166-0.677), and TG was a risk factor for CHD(OR=1.545, 95%CI: 1.007-2.371). Correlation analysis showed that Gensini score was negatively correlated with FT3(r=-0.488, P<0.05). There was no correlation between thyroid function and blood lipid.Conclusion FT3 is negatively correlated with coronary heart disease and its severity in perimenopausal women with non-low T3 syndrome. FT3 reduction may be one of the important factors affecting the incidence of coronary heart disease. It has very important clinical siginificance to detect thyroid function in perimenopausal patients with coronary heart disease.
