系统性红斑狼疮患者外周血单个核细胞中ADAR1与IFN-α的相关性及临床意义

Correlation and Clinical Significance of ADAR1 in PBMCs and IFN-α in Patients with Systemic Lupus Erythematosus

摘要 目的检测系统性红斑狼疮(SLE)患者外周血单个核细胞(PBMCs)中RNA腺苷脱氨酶1(ADAR1)与血清IFN-α的表达量,并分析二者的相关性及临床意义。方法选取2019年1-8月于郑州大学第一附属医院风湿免疫科住院治疗的30例SLE女性患者作为SLE组,选取同时期30名健康女性作为对照组,使用定量逆转录-聚合酶链反应(qRT-PCR)检测SLE患者PBMCs中ADAR1 mRNA的相对表达水平。采用酶联免疫吸附测定法(ELISA)检测血清中IFN-α表达水平。收集临床资料,评估患者的临床症状并计算SLE活动指数(SLEDAI),用SPSS 21.0软件分析ADAR1与IFN-α表达水平、SLEDAI、补体3(C3)、补体4(C4)和24 h尿总蛋白(24 h UTP)的相关性。结果 SLE组血清IFN-α[82.06(46.44,177.63)pg·mL-1](P<0.01)和PBMCs中ADAR1[0.73(0.27,1.54)](P<0.05)表达水平均高于对照组。当血清IFN-α水平<260.0 pg·mL-1时,SLE组PBMCs中ADAR1表达水平与IFN-α水平呈正相关(P<0.05),ADAR1表达水平与SLEDAI (P<0.01)及24 h UTP水平呈正相关(P<0.01),血清IFN-α表达水平与SLEDAI呈正相关(P<0.01),与C3呈负相关(P<0.05)。结论 SLE患者中异常表达的IFN-α可能参与调节ADAR1的表达,PBMCs中的ADAR1或可成为一个评估SLE疾病活动度的潜在生物标志物,为靶向治疗提供了理论基础。 Objective To explore the expression characteristics of adenosine deaminases acting on RNA-1(ADAR1) in peripheral blood mononuclear cells(PBMCs) and serum IFN-α of patients with systemic lupus erythematosus(SLE), and analyze their clinical significance.Methods Thirty SLE female patients who were hospitalized in the Department of Rheumatology and Immunology of the First Affiliated Hospital of Zhengzhou University from January 2019 to August 2019 were selected as the SLE group, and 30 healthy women were selected as the control group in the same period. The relative expression of the ADAR1 mRNA in PBMCs were determined by using quantitative reverse transcription polymerase chain reaction(qRT-PCR). ELISA technology was used to detect serum IFN-α expression levels. Collect clinical data, evaluate patients’ clinical symptoms and calculate SLE disease activity index(SLEDAI), the correlation between ADAR1 and serum IFN-α and other clinical indices of SLE patients were analyzed by Spearman correlation using SPSS 21.0 software.Results The expression levels of ADAR1 [0.73(0.27,1.54)](P<0.05) in PBMCs and serum IFN-α [82.06(46.44,177.63)pg·mL-1](P<0.01) of SLE group were higher than those of healthy group. When serum IFN-α levels in SLE patients <260.0 pg·mL-1, there was a positive correlation between ADAR1 expression in PBMCs and IFN-α levels(P<0.05). In addition, the expression levels of ADAR1 in PBMCs was positively correlated with SLEDAI(P<0.01) and 24-hour urine total protein(24 h UTP)(P<0.01), serum IFN-α of SLE group was positively correlated with SLEDAI(P<0.01) and negatively correlated with C3(P<0.05).Conclusion Abnormal IFN-α expression in SLE may be involved in the regulation of ADAR1 expression. ADAR1 in PBMCs may be a potential biomarker to evaluate the disease activity of SLE patients, which provides a theoretical basis for targeted therapy.
