宫颈癌放化疗效果与MMP-2基因多态性的关系分析 被引量:4

摘要 目的研究宫颈癌放化疗效果与基质金属蛋白酶2(MMP-2)基因单核苷酸多态性(SNPs)的关系。方法回顾性采集秦皇岛市第一医院收治的放化疗前有完整血样标本的97例宫颈癌患者的临床资料,提取白细胞DNA,聚合酶链式反应-限制性片段长度多态分析技术(PCR-RFLP)检测MMP-2-1306T位点SNPs基因型。所有患者均接受放化疗,评估3个月近期疗效,记录随访1年复发率。比较不同疗效/随访复发宫颈癌患者MMP-2-1306位点基因多态性的差异,筛选宫颈癌放化疗近期疗效的影响因素。结果携带MMP-2-1036 C/T基因型宫颈癌患者同步放化疗疗效分级优于携带C/C基因型患者(P<0.05);携带MMP-2-1036 C/C基因型患者随访复发率高于携带C/T基因型患者(P<0.05);不同肿瘤分期、不同分化程度、不同放疗剂量宫颈癌患者近期疗效分级比较差异有统计学意义(P<0.05);MMP-2-1036基因型、肿瘤分期、分化程度均为宫颈癌放化疗近期疗效的影响因素(P<0.05)。结论携带MMP-2-1036 C/C基因型宫颈癌患者对放化疗敏感性差,近期疗效欠佳,有较高的复发风险;MMP-2-1036位点基因型分布、肿瘤分期、分化程度均为宫颈癌放化疗效果的影响因素。 Objective To study the relationship between the differences in effects of radiotherapy and chemotherapy for cervical cancer and single nucleotide polymorphisms(SNPs)of matrix metalloproteinase 2(MMP-2)gene.Methods The clinical data of 97 patients with cervical cancer who had complete blood samples before radiotherapy and chemotherapy were collected.Leucocyte DNA was extracted and the SNPs genotype at MMP-2-1306T locus was detected by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP).All patients underwent radiotherapy and chemotherapy and the short-term curative effect in 3 months was evaluated.The recurrence rate during 1-year follow-up was recorded.Polymorphisms of MMP-2-1306 T loci in patients with cervical cancer and different curative effect/follow-up recurrence were compared.Influence factors of short-term effects of radiotherapy and chemotherapy for cervical cancer were screened.Results The grade of curative effect of concurrent radiochemotherapy in patients with cervical cancer and MMP-2-1036 C/T genotype was better than those with C/C genotype(P<0.05).The follow-up recurrence rate of patients with MMP-2-1036 C/C genotype was higher than that of patients with C/T genotype(P<0.05).There were statistically significant differences in the grade of short-term curative effect between patients with different tumor stages,different differentiation degrees and different radiotherapy doses(P<0.05).MMP-2-1036 genotype,tumor stage and differentiation degree were all factors that affect the short-term curative effect of radiotherapy and chemotherapy for cervical cancer(P<0.05).Conclusion Patients with cervical cancer carrying MMP-2-1036 C/C genotype have poor sensitivity to chemoradiotherapy,and the short-term curative effect is poor,with a high risk of recurrence.Genotype distribution at MMP-2-1036 locus,tumor stage and differentiation degree are factors affecting the effects of radiotherapy and chemotherapy for cervical cancer.
