芍药甘草汤加减治疗胃阴不足型消化性溃疡患者的临床效果及部分机制

Clinical and Some Mechanism Study on Modified Shaoyao Gancao Decoction on Gastroyin Deficiency Peptic Ulcer

目的:探讨芍药甘草汤加减对胃阴不足型消化性溃疡的疗效及作用机制。方法:选取2014年1月至2019年1月六安市中医院收治的胃阴不足型消化性溃疡患者80例作为研究对象,按照随机数字表法随机分为对照组和观察组,每组40例。对照组常规西药处理,观察组加用芍药甘草汤治疗,均治疗4周。观察2组炎性反应递质、T淋巴细胞亚群、胃肠激素、氧化应激、中医证候积分等变化并比较。结果:2组完成治疗后丙二醇(MDA)、内皮素-1(ET-1)、超氧化物歧化酶(SOD)、一氧化氮(NO)、胃泌素(GAS)、缩胆囊素(CCK)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)较治疗前均显著降低,且观察组优于对照组,差异有统计学意义(P<0.05)。2组治疗后CD3+、CD4+、CD4+/CD8+较治疗前均显著升高,CD8+较治疗前显著降低,差异有统计学意义(P<0.05),且观察组优于对照组,差异有统计学意义(P<0.05)。2组完成治疗后胃脘灼痛、口干少津、饥不欲食、手足心热、大便干结积分较治疗前显著下降,差异有统计学意义(P<0.05),且观察组显著低于对照组,差异有统计学意义(P<0.05)。结论:芍药甘草汤可改善胃阴不足型消化性溃疡,提高免疫功能,改善胃肠功能。

Objective:To investigate the efficacy and mechanism of modified Shaoyao Gancao Decoction in the treatment of peptic ulcer with stomach yin deficiency.Methods:A total of 80 cases of peptic ulcer with stomach yin deficiency who were adopted in Lu′an Hospital of Traditional Chinese Medicine from January 2014 to January 2019,were selected and randomly divided into a control group(n=40)and a study group(n=40).The control group was treated with routine Western medicine and the observation group was treated with Shaoyao Gancao Decoction for 4 weeks based on the control group.The changes of inflammatory factors,T lymphocyte subsets,gastrointestinal hormones,oxidative stress and TCM syndrome scores were observed and compared between the 2 groups.Results:1)After treatment,the levels of propylene glycol(MDA),endothelin-1(ET-1)superoxide dismutase(SOD),nitric oxide(NO),gastrin(GAS),cholecystokinin(CCK),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and interleukin-8(IL-8)in the study group were significantly lower than those before treatment,and the study group was superior to the control group.2)After treatment,CD3+,CD4+ and CD4+/CD8+ were significantly increased and CD8+ was significantly decreased in both groups,and the study group was superior to the control group(P<0.05).3)After treatment,the scores of epigastric burning pain,dry mouth and little fluid,hunger without appetite,hand and foot heart heat and dry stool in the 2 groups were significantly lower than those before treatment(P<0.05),and those in the study group were significantly lower than those in the control group(P<0.05).Conclusion:Shaoyao Gancao Decoction can improve peptic ulcer with stomach yin deficiency,enhance immune function and improve gastrointestinal function.
