Nora's lesion:a case report and literature review

通过报告1例左股骨旁Nora病,并复习相关文献,总结Nora病的临床及影像学表现特征。本例患者男性,41岁,病理检查提示左大腿骨、软骨及纤维组织,于骨骼肌内穿插生长,骨组织周围可见骨母细胞及破骨样巨细胞,软骨组织有异型,诊断为Nora病,又称为奇异性骨旁骨软骨瘤样增生。检索2009年10月1日~2019年10月1日期间报道的Nora病患者共计82例,其中,发生在手、足等短管状骨及其附近的有54例,余各部位均有发生的报道。在X线或CT上表现为不均匀高密度影,在MRI上T1为等、低信号,T2及压脂像为等、高信号,增强扫描可有强化。Nora病是一种罕见的良性骨膜外增生性病变,治疗上以手术切除为主,术后复发率较高,一般无恶变倾向。

The purpose of this article is to report the clinical and imaging characteristics of Nora's lesion by one-case re port and literature review.This patient was a 41-year-old male who suffered from Nora's lesion involving the left thigh and ad mitted into our hospital.The pathological examination revealed bone,cartilage,fibrous tissue,surrounded with osteoblasts and osteoclast-like cells in the lesion of muscle tissues on the left thigh with atypical chondral tissue,which was also termed as bi zarre parosteal osteochondromatous proliferation.In addition,a total of 82 cases of Nora's lesion were retrieved from October 12009 to October 12019.Of them,the small tubular bones of the hands and feet are involved in 54 patients,and other locations affected are also reported.The Nora's lesion presents as a heterogeneous high-density shadow in X-ray or CT,while usually hypo-isointense on T1-weighted sequences and iso-hyperintense on T2-weighted sequences and fat suppression sequences on MRI,which enhanced following contrast administration.The Nora's lesion is a rare benign exophytic proliferative lesion without malignant tendency,and surgical resection is the main treatment with higher recurrence rate postoperatively.
