肠瘘病儿肠内营养支持方案的实施与护理

Implementation and nursing of enteral nutrition support for children with intestinal fistula

目的:探讨肠瘘病儿实施肠内营养(EN)支持的护理经验与方法。方法:总结20例肠瘘病儿实施EN过程中的护理方法,途径的建立、输注的管理、联合消化液回输的管理是护理重点,结合瘘口管理、个体化功能锻炼与家庭式健康教育。结果:经(41.25±14.9)d的EN支持治疗及护理后,本组20例病儿的营养指标均得到明显改善,8例病儿行肠瘘切除肠吻合术治愈,12例行肠瘘胶堵自愈疗法获得成功。结论:EN的实施结合个体化护理,有利于肠瘘病儿营养状况的改善,对疾病预后起到促进作用。

Objective:To explore the nursing experience and methods of enteral nutrition(EN)support in children with intestinal fistula.Methods:This study summarized the nursing methods during the EN application of 20 cases with intestinal fistula.The key points of nursing were the establishment of the way,the management of infusion,and the management of fistuloclysis,which combined fistula management,individual functional exercise and family health education.Results:After(41.25±14.9)days of EN supportive treatment and nursing,the nutritional indexes of 20 fistula children were significantly improved.There were 8 children who had their fistulas cured by intestinal fistula resection and enterostomy,and 12 children who had self-sealing by glue application.Conclusion:The implementation of EN combining with individualized nursing is conducive to the improvement of the nutritional status of children with intestinal fistula and plays a role in promoting the prognosis of the disease.
