Experience of TAN Feng in Treating Migraine with Syndrome of Liver-yang Transforming into Wind

总结谭峰教授诊治肝阳化风型偏头痛的经验。谭峰教授认为偏头痛的发生与风邪作祟有关,以内伤为主,与肝尤为密切;认为其基本病机为肝阳化风、瘀血阻滞,并对肝阳化风型偏头痛提出了"三早"(早期病症结合,早期活血熄风,早期综合康复)诊治思路。早期的审证求因、病症结合能更好地指导临床中医精准治疗;偏头痛多因内生瘀血、无症之瘀所致,故须通过活血化瘀之品方能消散;配合情志调理、睡眠调整、针灸推拿等早期综合康复手段,亦可有效缓解偏头痛。谭峰教授常用天麻钩藤汤加减并酌加活血化瘀之药治疗肝阳化风型偏头痛,临床收效较显著。

The experience of Professor TAN Feng in treating migraine with the syndrome of liver-yang transforming into wind was summarized.It is suggested by Professor TAN Feng that the occurrence of migraine is correlated with the attack of wind pathogen,and migraine is characterized by the internal injury in particular the injury of liver.The basic pathogenesis of migraine is shown as liver-yang transforming into wind and blood stasis obstruction.Professor TAN Feng proposed that the thoughts for the diagnosis and treatment of migraine with the syndrome of liver-yang transforming into wind should follow the principles of early diagnosis by combining disease and symptom,early treatment with therapy of activating blood and subsiding wind,and early comprehensive rehabilitation.Early differentiating syndrome to identify cause and combination of disease and symptom are beneficial to the precise treatment with traditional Chinese medicine.Herbs for activating blood to resolve stasis can eliminate the endogenous blood stasis which shows no obvious manifestations in the patients with migraine.The early comprehensive rehabilitation therapy such as emotional regulation,improving sleep quality,and acupuncture and massage can effectively relieve the symptoms of migraine.The modification of Tianma Gouteng Decoction with the herbs for activating blood to resolve stasis prescribed by Professor TAN Feng for the treatment of migraine with the syndrome of liver-yang transforming into wind has achieved satisfactory effect in clinic.
