化痰行气消瘀法治疗阻塞性睡眠呼吸暂停低通气综合征的疗效及对血清IL-18、HIF-1α的影响

Therapeutic Effect of Applying Huatan Xingqi Xiaoyu Therapy in the Treatment of OSAHS and Its Influence on Se-rum IL-18and HIF-1α

目的:观察中药化痰行气消瘀法治疗阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的疗效及对血清白细胞介素-18(IL-18)和缺氧诱导因子-1α(HIF-1α)水平的影响。方法:将90例OSAHS患者按照随机数字表法分为2组,每组45例,两组均接受常规干预,对照组同时口服多索茶碱片,每次0.2g,每日2次,治疗组同时口服中药鼾证消组方,每日1剂,两组均连续治疗4周。比较两组治疗前后主要中医证候积分、多导睡眠检测(PSG)相关指标、血清IL-18、HIF-1α水平,并比较两组治疗总有效率。结果:治疗后治疗组主要中医证候睡时打鼾、鼾声停止呼吸暂停、白天嗜睡积分低于对照组(P<0.01);睡眠呼吸暂停低通气指数(AHI)、氧减指数(ODI)、氧饱和度<90%时间(T90)、微觉醒指数(MAI)低于对照组(P<0.01),平均氧饱和度(SaO_2)高于对照组(P<0.01);血清IL-18、HIF-1α水平低于对照组(P<0.01),治疗总有效率高于对照组(P<0.05),差异均有统计学意义。结论:化痰行气消瘀法可有效降低血清IL-18、HIF-1α水平,改善PSG指标和中医证候积分,治疗OSAHS效果显著,有临床推广应用价值。

Objective:To explore the therapeutic effect of applying Huatan Xingqi Xiaoyu Therapy in the treatment of ob-structive sleep apnea-hypopnea syndrome(OSAHS),and its influence on serum levels of interleukin-18(IL-18)and hypoxia inducible factor-1alpha(HIF-1alpha).Methods:Ninety patients with OSAHS were divided into two groups according to the digital random table method,with 45in each group.Both groups received routine intervention.The controlled group took Doxofyl-line Tablets orally,0.2g each time,twice a day.The treatment group also took the traditional Chinese medicine Hanzheng Xiao-zu Prescription orally,one dose a day.Both groups were treated continuously for 4weeks.The main TCM syndrome scores,PSG related indicators,serum levels of IL-18and HIF-1alpha were compared between the two groups before and after treatment,and the total effective rate of the two groups was compared.Results:After treatment,the scores of snoring,snoring apnea and day-time sleepiness of the treatment group were lower than those of the controlled group(P<0.01).The sleep apnea hypopnea index(AHI),oxygen reduction index(ODI),oxygen saturation<90%time(T90),micro-awakening index(MAI)were lower than those of the controlled group(P<0.01),and the average oxygen saturation(SaO2)was higher than that of the controlled group(P<0.01).The level of HIF-1alpha was lower than that of the controlled group(P<0.01),and the total effective rate was higher than that of the controlled group(P<0.05).The differences were statistically significant.Conclusion:Huatan Xingqi Xi-aoyu Therapy can effectively reduce the levels of IL-18and HIF-1a in serum,improve the PSG index and TCM syndrome inte-gral,so it has significant effect on OSAHS.It has clinical application value.
