自拟补脾益肠汤辅助治疗溃疡性结肠炎临床研究

Effect of Applying Self-made Bupi Yichang Decoction Adjuvant Therapy in the Treatment of Ulcerative Colitis

目的:观察自拟补脾益肠汤辅助治疗溃疡性结肠炎(UC)对患者病情转归及预后的影响。方法:选取2017年1月~2019年1月70例在我院肛肠科被确诊的UC患者,采用随机数表法随机分为对照组和观察组各35例,对照组采用常规治疗,口服美沙拉嗪肠溶片,观察组在对照组的基础上合用自拟补脾益肠汤,4周为1疗程,治疗两个疗程,在治疗前以及治疗两疗程后,分别观察两组的中医症候积分与症候疗效,内镜检测两组肠黏膜情况,血常规检测两组血红蛋白(Hb)、白蛋白(Alb)、血沉(ESR)、C反应蛋白(CRP)水平情况,ELISA检测两组炎性因子IL-1β、IL-8与TNF-α情况。结果:观察组疗效的总有效率高于对照组(P<0.01);观察组Hb水平较对照组无明显变化(P>0.05),观察组Alb水平较对照组明显降低(P<0.05),观察组ESR与CRP水平较对照组明显增高(P<0.01);治疗后观察组肠黏膜治疗有效率为高于对照组(P<0.01);治疗后观察组IL-1β、IL-8与TNF-α水平均低于对照组(P<0.01);观察组复发率远低于对照组(P<0.01)。结论:自拟补脾益肠汤配合美沙拉嗪治疗UC疗效显著,可缓解临床症状,帮助组织修复,且一定程度上防止复发,为UC患者的治疗提供借鉴。

Objective:To observe the effect of Self-made Bupi Yichang Decoction adjuvant therapy on disease outcomes and prognosis of patients with ulcerative colitis(UC).Methods:From January 2017to January 2019,70patients with UC who were diagnosed in the anorectal department of our hospital were selected and randomly divided into controlled group and observation group according to the random number table method,with 35cases in each group.The controlled group was given routine treat-ment and oral administration of Mesalazine Enteric-coated Tablets,and the observation group was combined with Self-made Bupi Yichang Decoction on the basis of the controlled group,and they were treated for two courses of treatment with 4weeks as one course.Before treatment and after 2courses of treatment,the TCM syndromes scores and symptom efficacy were observed in the two groups.Endoscopy was used to detect the intestinal mucosa in the two groups,and blood routine was used to detect the levels of hemoglobin(Hb),albumin(Alb),erythrocyte sedimentation rate(ESR)and C-reactive protein(CRP),and ELISA was used to detect the inflammatory factors IL-1β,IL-8and TNF-αin the two groups.Results:The total effective rate of efficacy in the observation group was higher than that in the controlled group(P<0.01).The level of Hb in the observation group was not significantly different from that in the controlled group(P>0.05),and the level of Alb in the observation group was significantly lower than that in the controlled group(P<0.05),and the levels of ESR and CRP in the observation group were significantly higher than those in the controlled group(P<0.01).The effective rate of intestinal mucosa treatment in the observation group was higher than that in the controlled group(P<0.01).After treatment,the levels of IL-1β,IL-8and TNF-αin the observation group were lower than those in the controlled group(P<0.01).The recurrence rate in the observation group was much lower than that in the controlled group(P<0.01).Conclusion:Self-made Bupi Yichang Decoction combined with Mesalazinehas has signif-icant efficacy in the treatment of UC,and it can alleviate clinical symptoms,help tissue repair,and prevent recurrence to some extent,and provide reference for the treatment of UC patients.
