儿童肝功能异常940例临床特征分析

Analysis of clinical features of abnormal liver function in 940 children

目的探讨940例肝功能异常患儿临床特征,为临床早期诊断、及时治疗提供参考依据。方法回顾性分析2015年1月~2018年9月来自云南地区以不明原因肝功能异常收治入院的940例患儿的临床资料,包括临床表现、病因、肝功能及临床疗效等。结果肝功能异常与年龄相关,以0~12个月婴儿为高发年龄群(706例,75.11%)。肝功能异常程度以轻度异常(534例,56.81%)最多见,其中以0~6个月小婴儿轻度肝功能异常最多见(P<0.05)。肝功能异常病因以感染为主(543例,57.76%,其中病毒感染353例,细菌感染190例),其次为其他不明原因(290例,30.85%)。经治疗88.40%的患儿治愈或好转。结论应重视儿童肝功能常规检查,特别是对于12个月以内的小婴儿。肝功能异常病因明确者应及时采取对因治疗;不明原因者需进一步完善遗传、代谢疾病基因筛查甚至行有创操作等检查以明确病因,早期干预,合理治疗。

Objective To explore the clinical characteristics of 940 children with abnormal liver function,so as to provide reference for early clinical diagnosis and timely treatment.Methods From January 2015 to September 2018,the clinical data of 940 children with unknown liver function abnormality admitted from Yunnan were analyzed retrospectively,including clinical manifestations,etiology,liver function and clinical efficacy.Results The abnormal liver function was related to age.The high incidence age group was 0-12 months old(706 cases,75.11%).The degree of liver function abnormality was mild(534 cases,56.81%)and mild(P<0.05).The main cause of liver dysfunction was infection(543 cases,57.76%,including 353 cases of virus infection and 190 cases of bacterial infection),followed by other unknown causes(290 cases,30.85%).After treatment,88.40%of the children were cured or improved.Conclusion We should pay attention to the routine examination of children's liver function,especially for infants within 12 months.If the cause of liver function abnormality is clear,timely treatment should be taken;if the cause is unknown,further improvement of genetic and metabolic disease gene screening and even invasive operation should be carried out to clear the cause,early intervention and reasonable treatment.
