自体肋软骨联合硅胶假体在短鼻畸形修复中的应用研究

Application of Autogenous Costal Cartilage Combined with Silica Gel Prosthesis in the Repair of Short Nose Deformity

目的:探讨自体肋软骨联合硅胶假体在短鼻畸形修复中的应用研究。方法:选择2015年2月-2017年3月笔者医院接诊的110例短鼻畸形患者进行研究,通过随机数表法分为观察组(58例)和对照组(52例),对照组采用硅胶假体修复,观察组在对照组的基础上联合自体肋软骨修复。比较两组手术情况、鼻修复效果、满意度及并发症情况。结果:观察组修复成功率高于对照组,手术时间长于对照组[100.00%vs92.31%,(89.73±11.05)minvs(56.60±5.28)min],差异有统计学意义(P<0.05);观察组术后12个月时鼻根高、鼻面角、鼻尖角均高于对照组[(6.21±0.38)mmvs(5.89±0.35)mm,(28.30±2.15)°vs(26.74±2.06)°,(89.25±3.03)°vs(87.01±2.85)°],差异有统计学意义(P<0.05)。观察组患者满意率高于对照组(100.00%vs92.31%),术后并发症总发生率低于对照组(3.45%vs15.38%),差异有统计学意义(P<0.05)。结论:自体肋软骨联合硅胶假体在短鼻畸形修复中效果显著,能有效改善鼻部形态,术后维持效果好,且可减少并发症发生,患者满意度高,值得在临床应用中推广。

Objective To explore the application of autogenous costal cartilage combined with silica gel prosthesis in the repair of short nose deformity. Methods 110 patients with short nose deformity who were admitted to our hospital from February 2015 to March 2017 were selected for study. They were divided into the observation group(58 cases) and the control group(52 cases) by random number table method. The control group was treated with silicone prosthesis. The observation group was treated with autogenous costal cartilage on the basis of the control group. The operation condition, nasal repair effect, satisfaction and complications were compared between the two groups. Results The success rate of the observation group was higher than that of the control group, and the operation time was longer than that of the control group[100.00% vs 92.31%,(89.73±11.05) min vs(56.60±5.28)min], the differences were statistically significant(P<0.05). The nasal root height, nasal facial angle and nasal tip angle of the observation group were higher than those of the control group at 12 months after operation[(6.21±0.38)mm vs(5.89±0.35)mm,(28.30±2.15)° vs(26.74±2.06)°,(89.25±3.03)° vs(87.01±2.85)°], the differences were statistically significant(P<0.05). The patients satisfaction rate of observation group was higher than that of control group(100.00% vs 92.31%, P<0.05). The total incidence of postoperative complications in the observation group was lower than that in the control group(3.45% vs 15.38%, P<0.05). Conclusion The effect of autogenous costal cartilage combined with silica gel prosthesis in the repair of short nose deformity is remarkable, it can effectively improve the shape nose, the maintenance effect is good, and can reduce the occurrence of complications, it is worthy of promotion in clinical application.
