难治性癫痫患者认知功能障碍的影响因素分析

Analysis of Influencing Factors of Cognitive Dysfunction in Patients with Refractory Epilepsy

目的分析影响难治性癫痫患者出现认知功能障碍的因素。方法选取2016年3月~2019年3月我院诊治的200例癫痫患者为研究对象,依据病情分为难治组(92例)和普通组(108例),比较两组一般资料(年龄、性别、病程、用药种类、癫痫发作频率、癫痫发持续时间)、认知功能障碍(即刻记忆、注意和计算力、回忆能力、语言能力、定向能力)评分(MMSE),分析难治组认知功能障碍危险因素。结果难治组病程、癫痫发持续时间、癫痫发作频率低于普通组,用药种类多于对照组(P<0.05),两组年龄、性别比较,差异无统计学意义(P>0.05);难治组MMSE评分均低于对照组(P<0.05);Logistic回归分析显示,用药种类多、癫痫发作频率高、癫痫发作持续时间长、病程长均是导致难治性癫痫患者出现认知功能障碍的独立危险因素。结论难治性癫痫患者普遍存在认知功能障碍,用药种类多、癫痫发作率高、癫痫发作持续时间、病程时间长可能是导致其认知功能障碍的危险因素。

Objective To analyze the factors affecting cognitive dysfunction in patients with refractory epilepsy.Methods 200 patients with epilepsy diagnosed and treated in our hospital from March 2016 to March 2019 were selected as the research subjects.They were divided into refractory group(92 cases)and ordinary group(108 cases)according to the condition,and compared the general information(age,gender,duration,type of medication,frequency of seizures,duration of epileptic seizures),cognitive dysfunction(immediate memory,attention and calculation,recall ability,language ability,orientation ability)scores between the two groups(MMSE),analyzed the risk factors of cognitive dysfunction in the refractory group.Results The duration,duration of seizures,and frequency of seizures in the refractory group were lower than those in the ordinary group.There were more medications than the control group(P<0.05).There was no significant difference in age and gender between the two groups(P>0.05);The MMSE scores of the refractory group were lower than those of the control group(P<0.05);Logistic regression analysis showed that there were many types of medication and the frequency of seizures was high,long duration of epileptic seizures,and long duration of disease are all independent risks leading to cognitive impairment in patients with refractory epilepsy factor.Conclusion Cognitive dysfunction is common in patients with refractory epilepsy.Many types of medications,high seizure rate,seizure duration and long duration may be risk factors for cognitive dysfunction.
