输尿管软镜联合钬激光碎石术在肾结石合并2型糖尿病中的效果

Effect of Soft Ureteroscope Combined with Holmium Laser Lithotripsy in Kidney Stones Combined with Type 2 Diabetes

目的观察输尿管软镜联合钬激光碎石术在肾结石合并2型糖尿病中的效果。方法选择2015年6月~2019年6月我院收治的肾结石患者134例,根据是否合并2型糖尿病分为实验组63例(肾结石合并2型糖尿病)和对照组71例(肾结石未合并2型糖尿病),两组均在全麻下接受输尿管软镜联合钬激光碎石术治疗。比较两组一般资料(年龄、性别、体重指数、HbA1-c、病程、结石直径、结石位置、肾功能)、临床指标[手术时间、住院时间、结石清除率、术后并发症(出血、感染)]。结果实验组HbA1c及体重指数高于对照组,差异有统计学意义(P<0.05);两组年龄、性别、病程、结石大小、位置、肾功能比较,差异无统计学意义(P>0.05),实验组和对照组术后1周的结石清除率分别为92.06%和91.55%,术后1个月的结石清除率分别为96.83%和95.77%,两组比较,差异无统计学意义(P>0.05)。两组手术时间、住院时间、出血,感染发生率比较,差异无统计学意义(P>0.05)。结论肾结石患者合并糖尿病并未影响输尿管软镜联合钬激光碎石术的治疗效果,结石清除率高且术后并发症可控。

Objective To observe the effect of soft ureteroscope combined with holmium laser lithotripsy in kidney stones combined with type 2 diabetes.Methods From June 2015 to June 2019,134 patients with renal stones were treated in our hospital.According to whether type 2 diabetes is combined,the experimental group is divided into 63 cases(kidney stones combined with type 2 diabetes)and 71 cases in the control group(without kidney stones combined with type 2 diabetes).Both groups received holmium laser lithotripsy under general anaesthesia surgery.The general data(age,gender,body mass index,hba1-c,course of disease,stone diameter,stone location,renal function),clinical indexes operation time,length of hospital stay,stone removal rate at 1 week and 1 month and postoperative complications(bleeding,infection)were compared between the two groups.Results The hba1-c and BMI of the experimental group were higher than those of the control group(P<0.05),there was no significant difference between the two groups in age,gender,course of disease,stone size,location and renal function(P>0.05),The stone removal rate in the experimental group and the control group at 1 week after operation was 92.06%and 91.55%,and the stone removal rate at 1 month after operation was 96.83%and 95.77%,respectively.There was no significant difference between the two groups(P>0.05).There was no significant difference in surgical time,length of hospital stay,and postoperative complications between the two groups(P>0.05).Conclusion Diabetes patients with diabetes mellitus have no effect on the treatment effect of soft ureteroscope combined with holmium laser lithotripsy,high stone clearance rate and controllable postoperative complications.
