摘要
目的探讨微量泵静脉注射乌拉地尔降压治疗对高血压性脑出血患者围术期脑血流的影响。方法回顾性分析2016年10月~2019年10月我院收治的83例高血压性脑出血患者临床资料,根据治疗方式不同分为实验组(n=42)和对照组(n=41)。对照组术前给予常规脑出血+降压治疗;实验组采用常规脑出血+乌拉地尔治疗。比较治疗后不同时间两组血压、心率及大脑中动脉血管血流动力学相关指标[收缩期峰值血流速度(Vs)、平均血流速度(Vm)、搏动指数(PI)及阻力指数(RI)]。结果治疗15 min后,实验组血压、心率水平均低于对照组,差异有统计学意义(P<0.05);治疗24 h后,两组Vs、RI、Vm、PI比较,差异无统计学意义(P>0.05)。结论采用微量泵静脉注射乌拉地尔治疗高血压性脑出血,有助于快速平稳血压,不增加心率,且不影响大脑中动脉血管血流动力学。
Objective To investigate the effect of micropump intravenous Urapidil anti hypertensive treatment on cerebral blood flow in patients with hypertensive intracerebral hemorrhage during perioperative period.Methods The clinical data of 83 patients with hypertensive intracerebral hemorrhage treated in our hospital from October 2016 to October 2019 were retrospectively analyzed and divided into experimental group(n=42)and control group(n=41)according to different treatment methods.The control group was given conventional cerebral hemorrhage+anti hypertensive therapy before operation;the experimental group was treated with conventional cerebral hemorrhage+Urapidil.Comparison of blood pressure,heart rate,and middle cerebral artery vascular hemodynamic indexes[peak systolic peak blood velocity(Vs),average blood velocity(Vm),pulsatility index(PI),and resistance index(RI)].Results After 15 min of treatment,the blood pressure and heart rate in the experimental group were lower than those in the control group,the differences were statistically significant(P<0.05).There was no significant difference in Vs,RI,Vm,and PI between the two groups after 24 h of treatment(P>0.05).Conclusion Intravenous injection of Urapidil by micropump for hypertensive intracerebral hemorrhage can help to stabilize blood pressure quickly,without increasing heart rate,and without affecting the middle cerebral artery vascular hemodynamics.
