单剂量静脉注射丙种球蛋白联合光疗治疗新生儿ABO溶血的效果

Efficacy of Single-Dose Intravenous Gamma Globulinin Injection Combined with Phototherapy on ABO Hemolytic Disease in Neonates

目的探讨单剂量静脉注射丙种球蛋白联合光疗对新生儿ABO溶血的临床效果。方法将80例新生儿ABO溶血患儿按随机数字表法分为对照组和观察组,每组40例。对照组患儿给予光疗治疗,观察组患儿给予光疗联合单剂量静脉注射丙种球蛋白治疗。观察2组治疗前后血清前清蛋白(PAB)与总胆汁酸(TBA)水平的变化情况,比较2组光疗时间、黄疸消退时间、住院时间及治疗期间不良反应发生率。结果2组治疗后血清PAB水平均较治疗前明显升高,血清TBA水平均较治疗前明显降低,且观察组治疗后血清PAB、TBA水平均较对照组改善更为显著,差异均有统计学意义(P<0.05);观察组光疗时间、黄疸消退时间、住院时间及不良反应发生率均较对照组明显减少,差异均有统计学意义(P<0.05)。结论单剂量静脉注射丙种球蛋白联合光疗可使新生儿ABO溶血患儿血清PAB水平升高、TBA水平降低,并减少不良反应,缩短临床症状改善时间。

Objective To investigate the effect of single-dose intravenous gamma globulin injection combined with phototherapy on ABO hemolytic disease in neonates.Methods Eighty neonates with ABO hemolytic disease were randomly treated with phototherapy alone(control group,n=40)or in combination with single-dose intravenous gamma globulin injection(observation group,n=40).Serum prealbumin(PAB)and total bile acid(TBA)levels were measured before and after treatment.Phototherapy duration,jaundice resolution time,hospital stay and adverse reactions were compared between the two groups.Results Serum PAB level increased and TBA concertration decreased after treatment in both groups(P<0.05).Furthermore,the improvement in PAB and TBA levels in observation group was more obvious than that in control group(P<0.05).In addition,compared with control group,phototherapy duration,jaundice resolution time,hospital stay and incidence of adverse reactions were significantly reduced in observation group(P<0.05).Conclusion Single-dose intravenous gamma globulin injection combined with phototherapy can increase PAB level,decrease TBA content,reduce adverse reactions and shorten the time to symptom improvement in neonates with ABO hemolytic disease.
