小剂量多巴胺联合呋塞米治疗心力衰竭的临床效果评价

Evaluation of clinical effect of low dose of dopamine combined with furosemide in the treatment of heart failure

目的:探讨小剂量多巴胺联合呋塞米治疗心力衰竭的临床效果。方法:纳入2016年5月-2019年2月江西省九江市德安县人民医院收治的心力衰竭患者60例,随机分为对照组和观察组各30例。对照组给予呋塞米治疗,20 mg+250 ml葡萄糖溶液静推,1次/d;观察组给予呋塞米联合小剂量多巴胺治疗,1.0μg·kg^-1·min^-2多巴胺静脉泵入。两组治疗2周。观察两组心功能参数、生化指标和不良反应情况。结果:治疗后,观察组心功能改善较好,与对照组相比差异有显著性(P<0.05);观察组的生化指标在Na^+、K^+方面,和对照组对比差异无统计学意义(P>0.05);而脑钠肽(BNP)水平则低于对照组(P<0.05)。两组不良反应对比差异无统计学意义(P>0.05)。结论:对于心力衰竭患者而言,采取小剂量多巴胺联合呋塞米治疗,可以获得较好预后,值得临床推广。

Objective:To investigate the clinical effect of low dose of dopamine combined with furosemide in the treatment of heart failure.Methods:Sixty patients with heart failure admitted to the People’s Hospital of Dean County,Jiujiang City,Jiangxi Province from May 2016 to February 2019 were selected and randomly divided into a control group and an observation group with 30 cases each.The control group was treated with furosemide,20 mg+250 ml glucose solution static push,1 time/d;and the observation group was treated with furosemide combined with low dose of dopamine,1.0μg·kg^-1·min^-2 dopamine intravenous pump.The treatment was 2 weeks for both groups.The cardiac function parameters,biochemical indexes and adverse reactions of the two groups were observed.Results:After treatment,the heart function of the observation group was improved better,and compared with that of the control group,the difference was significant(P<0.05);the biochemical indicators of the observation group were not significantly different from those of the control group in terms of Na^+and K^+(P>0.05);the brain natriuretic peptide(BNP)level of the observation group was lower than that of the control group(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion:For patients with heart failure,low dose of dopamine combined with furosemide can achieve a better prognosis,which is worthy of clinical promotion.
