复苏合剂对急性呼吸窘迫综合征患者机械通气的影响 被引量:4

摘要 目的:观察复苏合剂联合西医基础治疗对急性呼吸窘迫综合征(ARDS)患者机械通气的影响。方法:使用随机数字表法,将50例ARDS患者随机分为治疗组与对照组,每组各25例。纳入患者均给予西医基础治疗(积极控制原发病、肺保护性的机械通气策略、合理选择抗菌药物、循环支持、营养支持及镇静药物等),治疗组在此基础上加用复苏合剂,使用疗程14 d。比较两组患者使用研究药物后复测的动脉血氧分压(PaO2)、二氧化碳分压(PaCO2)、氧合指数(PaO2/FiO2)的变化,所需呼吸机支持时间,ICU和总住院时间。结果:两组患者治疗后PaCO2变化比较,差异无统计学意义;但PaO2、PaO2/FiO2与治疗前比较均改善,差异有统计学意义(P<0.01)。治疗组患者呼吸机支持时间、ICU住院时间和总住院时间均短于对照组,差异有统计学意义(P<0.05)。两组患者在治疗过程中均未发生严重不良反应。结论:中药复苏合剂可改善ARDS患者机械通气情况,延缓病情进展,改善患者预后。 Objective:To observe the effect of Fushu Mixture combined with conventional western medicine treatment on mechanical ventilation in patients with acute respiratory distress syndrome(ARDS).Methods:50 patients with ARDS were randomly divided into treatment group and control group with 25 cases in each group.All patients were given basic medical treatment(active control of primary disease,lung protective mechanical ventilation strategy,reasonable selection of antibacterial drugs,circulatory support,nutritional support and sedative drugs,etc.),and the treatment group added resuscitation mixture on this basis.It was expected to use the course for 14 days.Compared the changes in arterial partial pressure of oxygen(PaO2),partial pressure of carbon dioxide(PaCO2),oxygenation index(PaO2/FiO2),the required ventilator support time,ICU,and total hospitalization time in the two groups after using the study drug.Results:There was no significant difference in PaCO2 after treatment between the two groups of patients,but PaO2,PaO2/FiO2 improved compared with before treatment(P<0.01).The ventilator support time,ICU hospital stay,and total hospital stay of the treatment group were shorter than those of the control group,the differences were statistically significant(P<0.05).No serious adverse reactions occurred during the treatment of the two groups of patients.Conclusion:The Chinese medicine resuscitation mixture can improve the mechanical ventilation of patients with ARDS,delay the progression of the disease,and improve the prognosis of patients.
