关节镜下三隧道重建术治疗膝关节前交叉韧带损伤的疗效观察 被引量:13

摘要 目的:探讨关节镜下三隧道重建术对膝关节交叉韧带(ACL)损伤患者的治疗效果。方法:按数字表法将120例ACL损伤患者随机分为观察组(n=60)与对照组(n=60)。对照组给予Endobutton悬吊固定术治疗,观察组给予关节镜下三隧道重建术治疗。比较两组患者手术时间、术后下床活动时间、住院时间、负重行走时间等围术期情况;随访半年并比较两组患者膝关节功能(Lysolm、IKDC、Tengner评分)恢复情况,对两组患者术后膝关节疼痛评分(VAS)及负重行走时间进行比较。结果:两组患者手术时间比较,差异无统计学意义(P>0.05);观察组患者的术后下床活动时间、住院时间、负重行走时间短于对照组(P<0.05);治疗后,观察组Lysolm、IKDC、Tengner分值均高于对照组,VAS评分低于对照组,差异有统计学意义(P<0.05)。结论:关节镜下三隧道重建术治疗膝关节ACL损伤具有较高的疗效,可明显缩短患者住院时间,有效促进患肢早期功能锻炼,有利于膝关节功能的恢复。 Objective:To discuss the clinical effects of arthroscopic three tunnel reconstruction in the treatment of anterior cruciate ligament(ACL)injury.Methods:120 patients with ACL injury were randomly divided into the observation group(n=60)and the control group(n=60),the observation group were treated with arthroscopic three tunnel reconstruction,the control group were treated with endobotton suspension and fixation.The operation time,the time of getting out of bed activity,length of hospitalization and the time of weight-bearing walking were compared between the two groups,and followed up for a half year and compare the recovery of knee joint function(lysolm,IKDC and tengner scores)between the two groups,and the VAS pain score and weight-bearing walking time were contrasted between the two groups.Results:There was no significant difference in the operation time between the two groups(P>0.05).The time of getting out of bed,length of hospitalization and weight-bearing walking time in the observation group was significantly shorter than the control group after the operation(P<0.05).After the operation,lysolm,IKDC and tengner in the observation group were higher than those in the control group(P<0.05),but VAS score in the observation group was significantly lower than that in the control group,the differences were statistically significant(P<0.05).Conclusion:Arthroscopic three tunnel reconstruction has a high curative effect on ACL injury of knee joint,it can reduce the length of stay,effectively promote the functional exercise,it is beneficial to the recovery of knee joint function.
