IGF-IR与前列腺癌发生发展及预后的关系 被引量:8

摘要 目的探讨前列腺癌组织中胰岛素样生长因子受体Ⅰ(IGF-IR)表达与前列腺癌发生发展及预后的关系。方法选取本院获取的前列腺癌组织标本74例(癌组织)、因手术实施前列腺治疗的前列腺良性增生标本80例(BPH组);采用免疫组化技术检测两组标本中的IGF-IR蛋白表达情况,并分析前列腺癌组织中的IGF-IR阳性蛋白表达与前列腺癌特征及预后的关系,以及前列腺癌术后患者生存时间的影响因素。结果前列腺癌组织中的IGF-IR蛋白阳性表达率明显高于BPH组织,差异有统计学意义(P<0.001)。不同临床分期及淋巴结转移情况的前列腺癌组织中的IGF-IR蛋白阳性表达率差异有统计学意义(P<0.05)。IGF-IR蛋白阳性表达的患者术后生化复发生存率显著高于阴性表达者,总体生存率显著低于阴性表达者,差异均有统计学意义(P<0.05)。IGF-IR蛋白阳性表达和阴性表达对前列腺癌患者生化复发生存时间及总体生存时间的影响差异有统计学意义(P<0.05)。COX回归分析结果显示,临床分期、以及生化复发生存是影响患者生存状态的独立因素(P<0.05)。结论 IGF-IR蛋白表达增强可能会促进前列腺癌的发生及发展,但是否会对患者生存时间产生独立影响,仍有待进一步的大样本及长时间随访加以证实,IGF-IR或可成为前列腺癌治疗研究的新靶点。 Objective To explore the relationship between the expression of insulin-like growth factor receptorⅠ(IGF-IR)in prostate cancer tissue and the development and prognosis of prostate cancer.Methods 74 cases of prostate cancer tissues(cancer tissues)obtained from our hospital and 80 cases of benign prostatic hyperplasia due to surgery for prostate(BPH group) were recruited for this study.Immunohistochemical technique was used to detect the expression of IGF-IR protein in the two groups of specimens. The relationship between IGF-IR positive expression in prostate cancer tissues and the characteristics and prognosis of prostate cancer was analyzed. The factors affecting the survival time of patients with prostate cancer after surgery were also analyzed. Results The positive expression rate of IGF-IR protein in prostate cancer tissues was significantly higher than that in BPH tissues,and the difference had statistical significance(P<0.001). The positive expression rate of IGF-IR protein in prostate cancer tissues with different clinical stages and lymph node metastasis had statistical significance(P<0.05). The survival rate of postoperative biochemical recurrence in patients with positive expression of IGF-IR protein was significantly higher than that of those with negative expression,and the overall survival rate was significantly lower than that of those with negative expression,the difference had statistical significance(P<0.05).There was a statistically significant difference between the positive and negative expression of IGF-IR protein on the survival time of biochemical recurrence and the overall survival time of prostate cancer patients(P<0.001). COX regression analysis showed that clinical stage and survival of biochemical recurrence are independent factors affecting the survival status of patients(P<0.05). Conclusion The increased expression of IGF-IR protein may promote the development and progression of prostate cancer. However,whether it will have an independent effect on patient survival remains to be confirmed by further large samples and long-term follow-up. IGF-IR may consider as a new target for prostate cancer treatment.
