A-PRF在上颌窦内提升术中的临床应用

摘要 目的:探讨改良型富血小板纤维蛋白(advanced platelet-rich fibrin,A-PRF)在上颌窦内提升术中的临床应用。方法:选择我院上颌后牙剩余牙槽嵴高度不足的种植患者,26例42颗牙,采用Osstem TSⅢ型种植体系统,骨挤压与液压法联合经牙槽嵴入路内提升术,应用A-PRF作为植入物,并同期植入种植体。术后6-8月进行义齿修复。结果:术后曲面断层片显示上颌窦内提升幅度在3.3-7.8mm (平均6.1mm),26例患者42颗牙齿均取得良好的骨结合。结论:改良型富血小板纤维蛋白(A-PRF)是完全自体血液来源的组织,不存在排异反应,且具有诱导软硬组织形成作用,可作为一种有效的低成本的上颌窦提升生物材料应用于临床。 Objective: To investigate the clinical application of advanced platelet-rich fibrin(A-PRF) in intra-maxillary sinus lifting. Methods: 26 patients(42 teeth) with insufficient residual alveolar ridge height of maxillary posterior teeth of implant patients in our hospital were selected. Osstem TS Ⅲ implant system was adopted. Bone condensing and hydraulic lifting method were combined with internal lifting via alveolar ridge approach. A-PRF was used as implant and implant was implanted at the same time.Denture repair was performed from 6 to 8 months after operation. Results: Curved surface tomography showed that the elevation of maxillary sinus was 3.3 mm-7.8 mm(average 6.1 mm), and 42 teeth of 26 patients achieved good bone union. Conlusion: Advanced platelet rich fibrin(A-PRF) is a completely autogenous blood derived tissue, which has no foreign body reaction and can induce soft and hard tissue formation. It can be used as an effective and low-cost biomaterial for maxillary sinus lifting.
