床旁医用数字X射线摄影即时胸部摄片对心脏手术后患者的应用价值研究

Application value of instant chest radiography of bedside medical DR on patients post cardiac surgery

目的:评估床旁移动医用数字X射线摄影(DR)技术即时胸部摄片对心脏外科术后患者的应用价值。方法:选取医院收治的428例心脏外科术后患者,采用移动式床旁DR设备对其即时行胸部半卧前后位摄片,并对患者术后并发症、主动脉内气囊泵(IABP)置管成功率、漂浮导管(Swan-Ganz)置管成功率进行统计。结果:428例心脏术后患者床旁摄片结果显示,单侧或双侧胸腔积液291例,肺部感染165例,气胸26例,皮下气肿30例,单叶或节段性肺不张47例,气管插管位置过深或过浅18例,主动脉内气囊泵置管19例,漂浮导管置入223例。结论:床旁DR胸部摄影是全面了解心脏手术术后并发症的重要检查手段,同时可为临床及时准确而全面地评估心脏术后患者的病情提供影像学依据,可作为心脏手术术后常规检查必查项目。

Objective:To explore the effects of bedside mobile medical digital radiography(DR)technique on instant chest radiography of patients post cardiac surgery.Methods:428 patients who underwent cardiac surgery were selected.The mobile bedside DR equipment was adopted to implement instant photography for them on anteroposterior position of chest when they were semireclining.And the postoperative complication,the success rate of catheterization of intraaortic balloon pump(IABP)and the success rate of catheterization of Swan-Ganz of patients were calculated as statistic method.Results:Among 428 cases,291 cases had unilateral or bilateral pleural effusion,and 165 cases had pulmonary infection,and 26 cases occurred pneumothorax,and 30 cases occurred subcutaneous emphysema,and 47 cases occurred unilobar or segmental atelectasis,and the positions of trachea cannula of 18 cases were too deep or too shallow,and 19 cases underwent catheterization of IABP,and 223 cases underwent catheterization of Swan-Ganz.Conclusion:Bedside DR on chest is an important mean of examination for comprehensive understanding postoperative complications of cardiac surgery,and it can provide the imaging basis for timely,accurate and comprehensive assessing the condition of patients post cardiac surgery in clinical practice.It can be used as a necessary item of routine examination post cardiac surgery.
