雷公藤多苷联合氢化可的松治疗过敏性紫癜儿童的疗效研究 被引量:5

Clinical study of Tripterygium wilfordii polyglycosides combined with hydrocortisone in the treatment of children with allergic henoch-schonlein purpura

摘要 目的探究雷公藤多苷联合氢化可的松治疗过敏性紫癜儿童的临床效果。方法选择2015年3月至2017年8月解放军联勤保障部队第九四〇医院收治的过敏性紫癜儿童80例作为研究对象,依据随机数字表法分为对照组40例及观察组40例。其中对照组予以氢化可的松琥珀酸钠治疗,观察组在对照组治疗方案基础上联合雷公藤多苷进行治疗。对2组患儿治疗后临床疗效进行分析,观察2组患儿临床症状改善及不良反应发生情况,并检测2组治疗前后氧化应激指标的水平变化。结果治疗后,观察组临床有效率(92.5%)显著优于对照组(70.0%)(P<0.05)。治疗后,观察组紫癜消失时间、腹痛消失时间及关节肿痛消失时间均显著低于对照组(P<0.05);2组总抗氧化能力(T-AOC)及超氧化物歧化酶(SOD)水平均显著高于治疗前(P<0.05),丙二醛(MDA)水平显著低于治疗前(P<0.05),且观察组T-AOC及SOD水平均显著高于对照组(P<0.05),MDA水平显著低于对照组(P<0.05)。治疗过程中2组均未发生相关不良反应。结论雷公藤多苷联合氢化可的松治疗儿童过敏性紫癜具有较好的临床疗效,可有效缩短患儿的临床症状时间,同时改善患儿的氧化应激反应,安全性高,具有一定的临床推广价值。 Objective To investigate clinical effects of Tripterygium wilfordii polyglycosides combined with hydrocortisone in the treatment of children with allergic henoch-schonlein purpura.Methods Eighty children with allergic Henoch-Schonlein purpura admitted to our hospital from March 2015 to August 2017 were selected as research subjects.By random cluster sampling method,they were divided into the control group(n=40)and the observation group(n=40).The control group was treated with hydrocortisone sodium succinate,while the observation group received Tripterygium wilfordii polyglycosides on the basis of the treatment received by the control group.The clinical efficacy after treatment was analyzed and compared between the 2 groups.The improvement of clinical symptoms and adverse reactions of the 2 groups were closely observed,and the changes in oxidative stress indexes before and after treatment were detected for study.Results After treatment,the clinical effective rate of the observation group(92.5%)was significantly better than that of the control group(70.0%),with statistical significance(P<0.05).Following treatment,the disappearance time of purpura,abdominal pain and joint swelling and pain in the observation group were all significantly shorter than those in the control group(P<0.05);The levels of T-AOC and SOD in the 2 groups were significantly higher than those before treatment(P<0.05),while the levels of MDA were significantly lower than those before treatment(P<0.05),and the levels of T-AOC and SOD in the observation group were significantly higher than those in the control group(P<0.05),and the levels of MDA were significantly lower than those in the control group(P<0.05).There were no adverse reactions in the course of treatment.Conclusion Tripterygium wilfordii polyglycosides combined with hydrocortisone in the treatment of children with allergic Henoch-Schonlein purpura could produce good clinical efficacy,effectively shorten clinical symptoms,and at the same time improve the oxidative stress response of the children,with good safety.For this reason,it is worth further clinical popularization.
