基于复杂网络分析的中医药治疗冠心病合并抑郁用药规律

Medication Rule of Traditional Chinese Medicine in Treating Coronary Heart Disease with Depression: Based on Complex Network Analysis

目的总结中医药治疗冠心病合并抑郁的用药规律。方法检索中国知网、万方数据库、中文科技期刊数据库、中国生物医学文献数据库、Pub Med和Cochrane Library数据库中冠心病合并抑郁的临床研究文献,检索时间限定在1999年1月1日到2019年1月1日。采集纳入文献的处方及药物信息建立数据库,统计药物频次及药物功效分类,分析常用药物并对其进行复杂网络分析。结果最终获取43首方剂,共应用中药102味,出现总频次为482次,涉及补虚药、理气药、安神药、化痰药等19类药物。频次前10位中药为甘草(29)、柴胡(28)、茯苓(23)、丹参(20)、川芎(19)、郁金(17)、白芍(17)、枳壳(16)、当归(15)、酸枣仁(14)。通过复杂网络分析得出其常用药对为柴胡-甘草(19)、柴胡-川芎(16)、柴胡-郁金(15)、茯苓-甘草(15)、柴胡-白芍(15);其核心处方为柴胡、白芍、枳壳、甘草、丹参、当归、郁金、川芎、茯苓。结论中医药治疗冠心病合并抑郁主要以疏肝解郁、行气活血为主,常选用柴胡、白芍、枳壳、甘草、丹参、当归、郁金、川芎等药物,并常加用健脾益气或补益肝肾或养心安神等药物。

Objective To summarize the medication rule of traditional Chinese medicine(TCM) in treating coronary heart disease(CHD) with depression. Methods Clinical research literatures on CHD with depression from January 1,1999 to January 1,2019 were searched in Chinese National Knowledge Infrastructure(CNKI),Wanfang Database,China Journal Fulltext Database(CJFD),China Biology Medicine(CBM). The prescriptions and drug information included in the literature were used to establish a database. The drug frequency and drug efficacy classification was counted. The commonly used drugs were analyzed,and the complex network analysis was performed. Results A total of 43 prescriptions were obtained and 102 Chinese herbal medicines were obtained,with a total frequency of 482,involving 19 kinds of drugs for tonifying deficiency,regulating qi,tranquilizing mind and eliminating phlegm,etc. The top 10 most frequently used Chinese medicines were Gancao(Radix Glycyrrhizae,29),Chaihu(Radix Bupleuri,28),Fuling(Poria,23),Danshen(Radix Salviae Miltiorrhizae,20),Chuanxiong(Rhizoma Ligustici Chuanxiong,10),Yujin(Radix Curcumae,17),Baishao(Radix Paeoniae Alba,17),Zhiqiao(Fructus Aurantii,16),Danggui(Radix Angelicae Sinensis,15),Suanzaoren(Semen Ziziphi Spinosae,14). Complex network analysis showed that the common drug pairs were Radix Bupleuri with Radix Glycyrrhizae(19),Radix Bupleuri with Rhizoma Ligustici Chuanxiong(16),Radix Bupleuri with Radix Curcumae(15),Poria with Radix Glycyrrhizae(15),Radix Bupleuri with Radix Paeoniae Alba(15). The core prescriptions included Radix Bupleuri,Radix Paeoniae Alba,Fructus Aurantii,Radix Glycyrrhizae,Radix Angelicae Sinensis,Radix Curcumae,Ligustici Chuanxiong and Poria. Conclusion TCM treatment of CHD complicated with depression mainly relies on dispersing stagnated liver qi for relieving qi stagnation,promoting qi for activating blood circulation,and often uses Radix Bupleuri,Radix Paeoniae Alba,Fructus Aurantii,Radix Glycyrrhizae,Radix Angelicae Sinensis,Radix Curcumae,Ligustici Chuanxiong and other drugs. And herbs of invigorating spleen and replenishing qi,or tonifying and replenishing liver and kidney,or nourishing heart tranquillization were often used.
