医院社区一体化模式对糖尿病患者血糖管理的效果研究

Study on Effect of Hospital-Community Integration Model on Blood Glucose Management in Diabetic Patients

目的探讨医院社区一体化模式对糖尿病患者血糖的管理效果。方法选择2017年3月至2018年3月在台州市中医院诊疗的糖尿病患者134例,其中将2017年3月至2017年8月患者作为对照组,采用常规管理。2017年9月至2018年2月患者作为观察组,采用医院社区一体化模式。每组患者67例,两组医护人员均为同一批人。比较管理前后两组患者糖代谢变化、治疗依从性和对糖尿病知识评分。结果管理后,两组患者的空腹血糖(FPG)和糖化血红蛋白(HbA 1c)较管理前降低;且观察组患者的FPG和HbA 1c低于对照组;而合理用药、合理饮食、合理运动和定期监测血糖依从性高于对照组;患者对血糖监测方法、血糖控制方式及低血糖预防措施的评分高于对照组,差异均有统计学意义(p<0.05)。结论医院社区一体化模式对糖尿病患者管理效果良好,可提高患者对治疗的依从性和对糖尿病知识认知状况。

Objective To explore the effect of integrated hospital-community model on blood glucose management in patients with diabetes.Methods A total of 134 diabetic patients who were diagnosed and treated in Taizhou Traditional Chinese Medicine Hospital from March 2017 to March 2018 were selected.Among them,patients from March 2017 to August 2017 were taken as the control group(n=67)and given routine management.Patients from September 2017 to February 2018 were taken as the observation group(n=67)and managed with integrated hospital-community model.The medical staff for both groups were the same.Changes in glucose metabolism,treatment compliance,and diabetes knowledge scores were compared between the two groups of patients before and after management.Results After management,the fasting blood glucose(FPG)and glycated hemoglobin(HbA1 c)of the two groups of patients were lower than before the management;and the FPG and HbA1 c of the patients in the observation group were lower than those in the control group;and the rational use of drugs,reasonable diet,reasonable exercise and regular blood glucose monitoring compliance were higher than those of the control group.Patients scored higher on blood glucose monitoring methods,blood glucose control methods,and hypoglycemic preventive measures than the control group,and the differences were statistically significant(p<0.05).Conclusion The integrated hospital-community model has a good effect on the management of patients with diabetes,which can improve their compliance with treatment and knowledge of diabetes.
