河南省2010-2018年新报告HIV/AIDS流行特征分析

Characteristics of newly reported HIV/AIDS cases in Henan province,2010-2018

目的分析2010-2018年河南省新报告HIV/AIDS流行特征,为艾滋病防控工作提供参考依据。方法从我国艾滋病防治基本信息系统中下载2010年1月1日至2018年12月31日河南省新报告HIV/AIDS个案信息,应用Excel 2010软件整理数据、SPSS 23.0软件进行统计学分析。结果2010-2018年,河南省新报告HIV/AIDS 41223例,新报告HIV/AIDS数从2010年的3087人逐年增加至2018年的5910人。新报告HIV/AIDS以性传播为主,性传播构成比从2010年的50.6%(1564/3087)升至2018年的98.8%(5839/5910),异性性传播从2010年的44.9%(1387/3087)先升后降至2018年的65.7%(3885/5910),男男性行为传播从2010年的5.7%(177/3087)升至2018年的33.1%(1954/5910)。异性性传播HIV/AIDS中有非婚异性性行为的比例从2010年的69.4%(962/1387)升至2018年的91.7%(3562/3885)(趋势χ^2=657.802,P<0.001)。15~24岁青年、≥60岁老年新报告HIV/AIDS数逐年增多,青年以男男性行为传播为主(52.7%,2561/4856),老年以异性性传播为主(86.4%,5907/6833),且老年异性性传播HIV/AIDS中,以男性为主,男性HIV/AIDS数从2010年的122例增加到2018年的738例,历年男性构成比在82.2%~91.0%之间,女性构成比从2010年的9.0%(12/134)增加到2018年的17.8%(160/898)。结论2010-2018年河南省新报告HIV/AIDS数逐年增多,性传播成为主要传播途径,艾滋病防控重点应以控制非婚异性性行为传播和男男性行为传播为主,并关注青年和≥60岁老年人群的性传播。

Objective To analyze the epidemiological characteristics of newly reported HIV/AIDS cases in Henan province during 2010-2018,and to provide theoretical evidence for strategies on prevention and control.Methods Information about newly reported HIV/AIDS cases in Henan between January 1,2010 and December 31,2018 were collected from the National Comprehensive HIV/AIDS Information System.Excel 2010 and software SPSS 23.0 were used for data cleaning and statistical analysis.Results During 2010-2018,a total of 41223 HIV/AIDS cases were newly reported in Henan,and the number of newly reported HIV/AIDS cases increased from 3087 in 2010 to 5910 in 2018.Sexual contact was the predominant transmission route in newly reported HIV/AIDS cases,and the proportion of sexual transmission increased from 50.6%(1564/3087)in 2010 to 98.8%(5839/5910)in 2018.The proportion of heterosexual transmission firstly increased from 44.9%(1387/3087)in 2010 and then declined to 65.7%(3885/5910)in 2018,and the proportion of MSM behavior related transmission increased from 5.7%(177/3087)in 2010 to 33.1%(1954/5910)in 2018.Among the newly reported heterosexual transmitted cases,the proportion of cases with non-marital heterosexual behaviors increased from 69.4%(962/1387)in 2010 to 91.7%(3562/3885)in 2018(χ^2=657.802,P<0.001).The number of newly reported HIV/AIDS cases in young people aged 15-24 years and old people aged≥60 years increased year by year.The youth cases were mainly infected by MSM behavior transmission(52.7%,2561/4856),and heterosexual transmission was the main infection route of the elder cases(86.4%,5907/6833).Among the elder cases infected by heterosexual transmission,cases with non-marital heterosexual behaviors were mainly males,the number of male cases increased from 122 in 2010 to 738 in 2018,and sex composition ratio was between 82.2%and 91.0%,while the sex composition ratio of female cases was increased from 9.0%(12/134)in 2010 to 17.8%(160/898)in 2018.Conclusions From 2010 to 2018,the number of newly reported HIV/AIDS cases in Henan increased.Sexual transmission had become the main transmission route.AIDS prevention and control should focus on the control of non-marital heterosexual transmission and MSM behavior transmission,and to pay more attention to young people and people aged 60 years and above.
