肿瘤专科日间化疗病区六西格玛管理模式的应用与效果评价

Application and Effect Evaluation of Six Sigma Management Model in Day-care Chemotherapy Area of Oncology Department

目的探讨六西格玛管理模式在肿瘤专科日间化疗病房应用的效果。方法选择2019年1—6月入住某肿瘤专科医院日间化疗的患者420例,按照患者住院号随机分为对照组210例,观察组210例,观察组运用六西格玛管理法,对照组运用常规护理方法,分析两组患者静脉导管、输液等候时间及医护患满意度的差异。结果实施六西格玛管理6个月后,对照组药物外渗发生率为2.9%,观察组为1.4%,观察组明显低于对照组(P<0.05);干预后观察组等待输液时间由4.60±0.89 h降到了3.23±0.63 h,对照组由4.20±1.12 h下降到3.97±0.96 h,观察组实施后效果显著(P<0.05);观察组患者、医生、护士满意度得分分别为9.83±1.03,8.45±1.74,11.51±0.82,对照组患者、医生、护士的满意度得分分别为8.97±1.26,7.88±1.09,10.20±0.75,观察组明显高于对照组,差异具有统计学意义(P<0.05);但静脉管路堵塞或滑脱发生率差异无统计学意义(P>0.05)。结论六西格玛管理模式的应用能有效优化肿瘤日间化疗病房管理,提高护理质量以及患者、医生及护士的满意度。

Objective To explore the effect of the Six Sigma management model in the daytime chemotherapy ward of the oncology department.Methods From January to June 2019,420 patients who were admitted to a cancer specialty hospital during daytime chemotherapy were randomly divided into control group(n=210)and observation group(n=210)according to their hospitalization number.The observation group used the Six Sigma management method,and the control group used conventional nursing methods.The differences in the venous catheter,infusion waiting time and the satisfaction of medical care patients were analyzed.Results Six months after the implementation of Six Sigma management,the incidence of drug extravasation in the control group was 2.9%,that in the observation group was 1.4%,and the difference was statistically significant(P<0.05).The waiting time for infusion in the observation group after intervention fell from 4.60±0.89 to 3.23±0.63,the control group decreased from 4.20±1.12 to 3.97±0.96,with more significant effect observed in the observation group after implementation(P<0.05).The satisfaction scores of patients,doctors and nurses in the observation group were 9.83±1.03,8.45±1.74,and 11.51±0.82,the satisfaction scores of patients,doctors and nurses in the control group were 8.97±1.26,7.88±1.09,and 10.20±0.75.The observation group scored significantly higher than the control group and the difference was statistically significant(P<0.05).However,there was no statistically significant difference in the incidence of pipeline blockage or slippage(P>0.05).Conclusion The application of the Six Sigma management model can effectively optimize the management of tumor daytime chemotherapy wards,improve the quality of care and the satisfaction of patients,doctors and nurses.
