我院分剂量药学门诊对儿科分剂量用药的影响

Effect of Dosage Division Service in Pharmacy Ambulatory Clinic on Pediatric Low-Dosage Administration in Our Hospital

目的调查我院儿科门诊片剂分剂量使用情况,探讨分剂量药学门诊对儿科分剂量用药的影响,为保障儿童用药安全提供参考。方法通过医院信息管理系统(HIS)调取2019年8月儿科门诊处方数据进行统计分析,随机筛选40位患儿作为对照组,并在分剂量药学门诊中随机筛选40位患儿作为观察组,其中观察组予以药学干预,对照组不作干预。定期进行药学随访。结果2019年8月儿科门诊片剂分剂量处方占当月总处方量的17.48%,其中片剂分剂量为成人规格的1/2片占81.17%,低于成人规格的1/4片占4.79%。分剂量药学门诊儿科片剂分剂量药品共计142例,片剂分剂量低于成人规格的1/4片占88.73%。用药依从性方面,观察组较干预前有显著提高(P=0.045),对照组干预前后差异无统计学意义(P=0.478)。结论分剂量药学门诊由专业药师负责片剂精准的分剂量操作,为患儿提供全程化药学服务,指导合理用药,保障用药安全,有效提高患儿用药依从性。

OBJECTIVE This study was designed to investigate the condition of tablet splitting in pediatric outpatient department of our hospital,and to explore the influence of dosage division service in our ambulatory clinic on pediatric low-dosage administration so as to provide reference to ensure the safety of childrens medication.METHODS The data of prescription of pediatric outpatient department in August 2019 were collected and statistically analyzed in the hospital information management system(HIS),40 children were randomly selected as control group,and 40 children were randomly selected as observation group in divided dose pharmaceutical outpatient service.The observation group received pharmaceutical care whereas the control group did not.Regular follow-up was performed in both groups.RESULTS Prescription including tablet splitting accounted for 17.48%of total prescription of pediatric outpatient department in August 2019.81.17%of which was 1/2 of adult dosage and 4.79%of which was less than 1/4 of adult dosage.A total of 142 tablet splitting cases of pediatric tablets were recorded in pharmacy ambulatory clinic,88.73%of which were less than 1/4 of adult dosage.The compliance improved significantly in observation group after intervention(P=0.045).Such improvement was not observed in control group(P=0.478).CONCLUSION Dosage division service in pharmacy ambulatory clinic is to split tablets precisely by professional pharmacists,which can provide the whole process pharmaceutical care for children,guide the rational use of drugs,ensure drug safety,and improve pediatric patient compliance effectively.
