手术室细节护理在肾癌后腹腔镜根治性肾切除术安全管理中的应用

Efficacy of meticulous nursing in operating room and safety management in laparoscopic radical nephrectomy for renal cancer

目的探讨手术室细节护理在肾癌后腹腔镜根治性肾切除术安全管理中的应用效果。方法选取2017年1月至2019年12月间驻马店市中心医院收治的行经腹腔镜根治性肾切除术治疗的96例肾癌患者,根据采用的护理方法不同进行分组,其中,采用手术室常规护理的48例患者纳入对照组,采用手术室细节护理的48例患者纳入观察组。比较两组患者护理质量及安全、护理满意度、患者术后疼痛感受及术后恢复状况。结果观察组患者消毒麻醉、器械准备、设备管理、医护配合以及护理安全评分均高于对照组,差异均有统计学意义(均P<0.05)。观察组患者护理态度、责任、适感以及护患关系评分均高于对照组,差异均有统计学意义(均P<0.05)。观察组患者术后24h、48h疼痛评分均低于对照组,差异均有统计学意义(均P<0.05)。术后72h时,两组患者疼痛评分比较,差异无统计学意义(P>0.05)。观察组患者尿道管拔除、引流管拔除、下床活动和住院时间均短于对照组,差异均有统计学意义(均P<0.05)。结论在肾癌后腹腔镜根治性肾切除术安全管理中,手术室细节护理应用效果较好,可有效提高手术室护理质量及安全,提升患者护理满意度,改善护患关系,且有利于患者术后恢复,具有临床应用价值。

Objective To explore the efficacy of meticulous nursing in operating room and safety management in laparoscopic radical nephrectomy for renal cancer.Methods From January 2017 to December 2019,96 patients who underwent laparoscopic radical nephrectomy for renal cancer at Zhumadian Central Hospital were selected and divided into an observation group and a control group based on the nursing they received with 48 patients in each group.The observation group received meticulous nursing in the operating room and the control group received conventional nursing in the operating room.The two groups were compared for nursing quality and safety,patient satisfaction with nursing,postoperative pain,and postoperative recovery.Results Scores of disinfection and anesthesia,equipment preparation,equipment management,medical care and nursing safety scores were significantly higher in the observation group than in the control group in(all P<0.05).In terms of nursing attitudes,responsibilities,fitness and relationship between nurses and patients,were statistically significant,the observation group were significantly higher than the control group(all P<0.05).Pain score was significantly lower in the observation group than in the control group at 24 h and 48 h after the surgery(all P<0.05).There was no significant difference in pain score between the two groups at 72 h after the surgery(P>0.05).Time to extubation,time to out-of-bed activity,and length of hospital stay were significantly shorter in the observation group than in the control group(all P<0.05).Conclusion Efficacy of meticulous nursing in operating room and safety management is significant,which can improve nursing quality and safety in the operating room,patient satisfaction with nursing,and the relationship between patient and nurse in patients undergoing laparoscopic radical nephrectomy for renal cancer.It deserves the clinical expansion.
