焦虑症从魄论治探析

Treatment of anxiety from TCM theory of corporeal soul

焦虑症又称焦虑性精神障碍,以与现实不相称的、广泛和持续性的担忧、紧张不安或反复发作的惊恐为主要特征,常伴有心悸、胸闷、气短、出汗、紧张性震颤、口干及颜面潮红或苍白等躯体症状。中医认为五脏神在调节机体精神情志等方面具有重要作用。魄属于五脏神的理论范畴,魄的生理功能包括调节个体的精神情志、感觉运动以及内脏功能等。魄的功能正常依赖于肾精充足、肺气充沛、气机畅通。魄的功能失常可导致焦虑症精神症状以及躯体症状的出现。魄功能失调所导致的焦虑症,主要病机包括因肺气郁滞而导致的魄不畅以及因肾精亏虚、肺气不足而导致的魄不宁。可由肺肾入手调节魄的功能,从宣肺畅魄、益肾强魄、补肺涵魄等方面对焦虑症进行辨证施治。

Anxiety disorder is characterized by recurrent panic or widespread and persistent anxiety and nervousness which are not commensurate with reality. It is often accompanied by physical symptoms such as palpitations, chest tightness, shortness of breath, sweating, tension tremor, a dry mouth and flushing or a pale face. Chinese medicine believes that the spirits in five internal organs have an important role in regulating the mind and emotions. The corporeal soul belongs to the theoretical category of the spirits in five internal organs. The physiological functions of the corporeal soul include regulating the individual’s emotions, perceptions, movements and internal organ functions. The normal operation of corporeal soul depends on abundance of kidney essence, plentiful qi in the lungs and unobstructed qi movement. The abnormal function of corporeal soul can cause mental symptoms and physical symptoms of anxiety. The main pathogenesis of anxiety caused by dysfunction of corporeal soul includes stagnation of qi, inadequate kidney essence and insufficient lung qi. Therefore, we can replenish kidney essence, supplement lung qi and regulate the qi movement to regulate the function of corporeal soul and treat anxiety according to syndrome differentiation.
