改良三伏天灸药方治疗小儿过敏性鼻炎的优越性研究

Clinical Study on Superiority in the Treatment of Allergic Rhinitis in Children by Modified Sanfu Tianjiu Prescription

目的:从改良药方及调配方法、精简并固定穴位、加强天灸前后护理等方面研究改良三伏天灸药方治疗小儿过敏性鼻炎的优越性,为临床避免口服药物的毒副作用及其他外治法引起的疼痛、繁琐等造成依从性差等问题提供一种可靠的治疗方案。方法:将符合纳入标准的过敏性鼻炎患儿100例随机分为3组,天灸组34例采用改良的药方进行三伏天灸治疗;西药组32例采用口服酮替芬结合普通药方三伏天灸治疗;贴敷组34例选用普通药方三伏天灸治疗,时间为2017年三伏时间,每10天1次,每次贴敷1 h,共治疗5次。观察并比较3组患儿治疗前后症状、体征、VAS评分情况及IgE的含量。结果:1)天灸组中显效17例,有效14例,无效3例,总有效率为91.18%;西药组中显效7例,有效16例,无效9例,总有效率为71.88%;贴敷组中显效2例,有效11例,无效21例,总有效率为38.24%。2)与治疗前比较,治疗后在症状评分及VAS评分方面天灸组、西药组、贴敷组之间比较均有统计学意义(P<0.01,P<0.05),与疗效结果表现一致,且两项评分中表现出一定的相关性。3)治疗后在体征评分及血清IgE水平方面比较,天灸组优于贴敷组(P<0.05)。结论:改良三伏天灸药方治疗小儿过敏性鼻炎的用药、选穴操作简便,治疗用时短,安全、快捷,无痛、无毒副反应,能明显提高患儿及家长的依从性,临床疗效显著,即时疗效快。

Objective:To research the advantages of modified Sanfu Tianjiu prescription in the treatment of allergic rhinitis in children from the aspects of modifying prescription and preparing method,simplifying fixed acupoints,and strengthening care before and after moxibustion,thus to provide a reliable treatment for the disease by avoiding side effects of oral drugs and the poor compliance caused by pain or time consuming.Methods:100 cases met the inclusion criteria were randomly divided into the modified Tianjiu group(n=34,modified prescription),the western medication group(n=32,Ketotifen+common Tianjiu)and the common Tianjiu group(n=34,common prescription).The duration of treatment was once every 10 days in the dog days of 2017,each time lasted for one hour and was treated for five times.The curative effect was observed after the treatment.Results:There were 17 excellent cases,14 effective cases and 3 invalid cases in the modified Tianjiu group,the total effective rate was 91.18%.There were 7 excellent cases,16 effective cases and 9 invalid cases in the western medication group,the total effective rate was 71.88%.There were 2 excellent cases,11 effective cases and 21 invalid cases in the common Tianjiu group,the total effective rate was 38.24%.There were statistical differences in symptom scores and VAS scores among the modified Tianjiu group,the western medication group and the common Tianjiu group(P<0.01,P<0.05);which was consistent with above clinical effects,also there was a certain correlation between the evaluation of symptom scores and VAS scores.The modified Tianjiu group was superior to the common Tianjiu group in terms of sign scores and IgE level after the treatment(P<0.05).Conclusion:Modified Sanfu Tianjiu prescription has a remarkable clinical effect on children with allergic rhinitis.It has an immediate effect and can significantly improve the compliance of children and parents.It is also easy to prepare the Tianjiu prescription and to select acupoints,this therapy only takes less time with high safety.
