电针腕背四穴治疗中风后腕手功能障碍临床研究

Clinical Study on Electro-Needling Four Points of Dorsal Wrist in the Treatment of Post-Stroke Hand-Wrist Dysfunction

目的:评价电针腕背四穴治疗中风后腕手功能障碍的临床疗效。方法:将符合纳入标准的60例患者,随机分为对照组、治疗组,各30例,对照组给予外关、肩贞、曲池、手三里、合谷为主治疗,治疗组给予阳溪、中泉、阳池、阳谷四穴电针治疗,1次/d,6 d为一疗程,疗程间隔1 d,连续治疗观察4个疗程,以FMA评分、Lindmark评分、Brunnstrom偏瘫手运动功能等级、ADL评分进行效果评判。结果:治疗后两组均可改善FMA评分、Lindmark评分、Brunnstrom偏瘫手运动功能等级、ADL评分,与治疗前比较差异具有统计学意义(P<0.05),两组比较差异具有统计学意义(P<0.05);治疗组总有效率86.67%(26/30),高于对照组63.33%(19/30),差异具有统计学意义(P<0.05)。结论:电针腕背四穴可有效改善中风患者手腕部的运动、感觉功能,改善患者的偏瘫状态、提高患者日常生活质量,值得临床推广。

Objective:To evaluate the clinical efficacy of electro-needling the four points of dorsal wrist in the treatment of post-stroke hand-wrist dysfunction.Methods:60 patients who met the inclusion criteria were randomly divided into the control group and the treatment group,with 30 cases in each group.The control group was treated with needling Waiguan(SJ5),Jianzhen(SI9),Quchi(LI11),Shousanli(LI10)and Hegu(LI4).The treatment group was treated with electro-needling Yangxi(LI5),Zhongquan(EX-UE3),Yangchi(SJ4)and Yanggu(SI5)these four dorsal wrist points.The treatment was once a day,6 days for one course of treatment with one day interval,and the whole treatment was continuous four courses.FMA score,Lindmark score,Brunnstrom hemiplegia hand function level and ADL score were used to evaluate efficacy.Results:FMA score,Lindmark score,Brunnstrom hemiplegia exercise function grade and ADL score were all improved after the treatment in the two groups(P<0.05),the improvements in the treatment group were more significant(P<0.05,P<0.01).The total effective rate of the treatment group was 86.67(26/30)%,which was significantly higher than 63.33%(19/30)of the control group(P<0.05).Conclusion:Electro-needling the four points of the dorsal wrist can effectively improve the movement and sensory function of the wrist of stroke patients,it can improve the patient's hemiplegia and improve the life quality of the patients.The therapy is worthy of clinical promotion.
