改良指套球囊法与iupu法建立后腹膜腔在泌尿外科后腹腔镜手术中的应用比较

Application comparison between modified fingerstall balloon method and iupu method in establishment of retroperitoneal cavity during urological retroperitoneal laparoscopic surgery

目的观察改良指套球囊法与iupu法建立后腹膜腔在泌尿外科后腹腔镜手术中的应用效果。方法本院泌尿外科收治的130例后腹腔镜手术患者,2016年6月至2017年12月收治的65例纳入对照组(iupu法建立后腹膜腔),2018年1月至2019年6月收治的65例纳入观察组(改良指套球囊法建立后腹膜腔),比较两组建立后腹膜腔有关指标、并发症发生情况、建腔满意度及尿常规。结果两组成本费用占总费用百分比均较低,但与对照组比较,观察组建立后腹膜腔时间短,建腔操作中出血量少,且一次性成功建腔率明显高(P<0.05),观察组并发症发生率明显低于对照组(P<0.05),建腔满意度评分明显高于对照组,而蛋白尿、尿白细胞及尿红细胞检出率明显低于对照组(P<0.05)。结论与iupu法比较,改良指套球囊法在泌尿外科后腹腔镜手术应用中有建腔时间短、损伤小、成本低、建腔成功率、使用满意度高且安全性好等明显优势,有一定临床推广应用优势。

Objective To observe the application effect of modified fingerstall balloon method and iupu method in the establishment of retroperitoneum in urological retroperitoneal laparoscopic surgery.Methods One hundred and thirty patients undergoing retroperitoneal laparoscopic surgery at department of urology surgery in our hospital were enrolled in the study.Of these patients,65 patients admitted between June 2016 and December 2017 were included in control group(establishing the retroperitoneum by iupu method),and 65 patients admitted between January 2018 and June 2019 were included in observation group(establishing the retroperitoneum by modified fingerstall balloon method).The related indexes of retroperitoneum establishment,incidence of complications,satisfaction with retroperitoneum establishment and urine routine were compared between the two groups.Results Compared with the control group,the observation group had a shorter time to establish retroperitoneal cavity,less bleeding and a higher rate of one-time successful cavity construction(P<0.05).The incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).The satisfaction score of retroperitoneum establishment in the observation group was significantly higher than that in the control group,and detection rates of proteinuria,urine leukocyte and urine erythrocyte were significantly lower than those in the control group(P<0.05).Conclusion Compared with iupu method,there are obvious advantages of modified fingerstall balloon method in urological retroperitoneal laparoscopy,including short retroperitoneum establishment time,little damage,low cost,high success rate of retroperitoneum establishment and user satisfaction,and good safety.There are certain advantages in clinical promotion.
