骨水泥型加长柄髋关节置换结合股骨近端重建治疗老年不稳定性股骨转子间骨折的临床效果

Clinical effect of cemented lengthened shank hip replacement combined with proximal femur reconstruction in the treatment of senile unstable intertrochanteric fracture

目的探讨骨水泥型加长柄髋关节置换结合股骨近端重建治疗老年不稳定性股骨转子间骨折的临床效果。方法将40例老年不稳定性股骨转子间骨折患者随机分为骨水泥型加长柄髋关节置换结合股骨近端重建治疗组(研究组,n=20)和股骨近端防旋髓内钉治疗组(对照组,n=20)。分析两组临床疗效。结果研究组手术时间、切口长度长于对照组,术中出血量多于对照组,独立负重时间短于对照组,术后并发症总发生率低于对照组(P<0.05)。术后,研究组Harris评分高于对照组(P<0.05)。结论骨水泥型加长柄髋关节置换结合股骨近端重建治疗老年不稳定性股骨转子间骨折的临床效果较股骨近端防旋髓内钉显著。

Objective To explore the clinical effect of cemented lengthened shank hip replacement combined with proximal femur reconstruction in the treatment of senile unstable intertrochanteric fractures.Methods Forty patients with senile unstable intertrochanteric fracture were divided into cemented lengthened shank hip replacement combined with proximal femur reconstruction treatment group(study group,n=20)and proximal anti rotation intramedullary nail treatment group(control group,n=20).The clinical efficacy of the two groups were analyzed.Results The operation time and incision length of the study group were longer than those of the control group,the amount of intraoperative bleeding was higher than that of the control group,the time of independent weight-bearing was shorter than that of the control group,the total incidence of postoperative complications was lower than that in the control group(P<0.05).After operation,Harris score of the study group was higher than that of the control group(P<0.05).Conclusion The clinical effect of cemented lengthened shank hip replacement combined with proximal femur reconstruction in the treatment of senile unstable intertrochanteric fractures is more significant than proximal anti rotation intramedullary nail.
