社区老年高血压患者同型半胱氨酸的性别差异及与血脂的相关性研究

Gender Differences of Homocysteine and Its Correlation with Blood Lipid in Elderly Community Hypertension Patients

目的:探讨社区老年高血压患者同型半胱氨酸(Hcy)的性别差异,并分析其与血脂的相关性。方法:选取2019年1-12月在北京市东城区东花市社区卫生服务中心就诊的高血压患者852例,根据性别分为男性355例和女性497例,检测并比较两组Hcy、体质指数(BMI)、尿酸(UA)、血糖、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)水平。采用Spearman相关法分析不同性别患者Hcy与各临床资料的相关性;采用多因素线性回归分析不同性别患者Hcy的影响因素。结果:不同人群年龄、Hcy、TC、LDL-C、HDL-C、TG及UA比较,差异均有统计学意义(P<0.05)。女性人群Hcy和UA水平均低于男性,年龄、TC、LDL-C、HDL-C及TG均高于男性人群(P<0.05)。Spearman相关分析显示,总人群中Hcy与年龄、TG、UA均呈正相关,与LDL-C呈负关性(P<0.05);男性人群中Hcy与年龄、TG、UA均呈正相关,与TC呈负关性(P<0.05);女性人群中Hcy与年龄、HDL-C、TG、UA均呈正相关(P<0.05)。多元线性回归分析结果显示,年龄、TG及UA是总人群、男性人群及女性人群共同的影响因素(P<0.05)。结论:社区老年女性高血压患者Hcy水平低于男性,年龄、TG及UA可能是男性、女性高血压患者共同的影响因素。

Objective:To explore the gender differences of homocysteine(Hcy)in elderly community hypertensive patients,and to analyze its relationship with blood lipid.Method:A total of 852 patients with hypertension were selected from January to December 2019 in Donghua Community Health Service Center in Dongcheng District of Beijing.According to the gender,all subjects were divided into male(n=355)and female(n=497),and the level of Hcy,BMI,UA,blood glucose,TG,TC,HDL-C and LDL-C between two groups were investigated and compared.Spearman correlation method was used to analyze the correlation between Hcy and clinical data in different genders,and multivariate linear regression was used to analyze the influencing factors of Hcy.Result:The differences were statistically significant on age,Hcy,TC,LDL-C,HDL-C,TG and UA in different populations(P<0.05).The Hcy and UA levels were lower,the age,TC,LDL-C,HDL-C,and TG levels were higher in the female population than those in the male population(P<0.05).Spearman correlation analysis showed that Hcy was positively correlated with age,TG and UA,and negatively correlated with LDL-C in the total population(P<0.05).Hcy was positively correlated with age,TG and UA,and negatively correlated with TC in the male population(P<0.05).Hcy was positively correlated with age,HDL-C,TG and UA in the female population(P<0.05).Multiple linear regression analysis showed that age,TG and UA were the common influencing factors of the total population,male population and female population(P<0.05).Conclusion:The Hcy level in females elderly community hypertension patients is lower than that in males,and age,TG and UA may be the common influencing factors of hypertension patients in males and females.
