小脑顶核电刺激治疗慢性主观性头晕的效果

Effect of cerebellar fastigial nucleus electrical stimulation treating chronic subjective dizziness

目的探讨电刺激小脑顶核疗法对慢性主观性头晕患者的疗效。方法选取2018年11月~2019年12月我院收治的58例慢性主观性头晕患者作为研究对象,采用随机数字表法分为研究组29例和对照组29例。对照组给予常规抗抑郁药物治疗,而研究组在对照组基础上给予小脑顶核电刺激疗法。采用眩晕残障程度评定量表(DHI)和医院焦虑抑郁量表(HADS)进行评估。结果治疗前,两组患者各项指标比较,差异无统计学意义(P>0.05)。治疗30 d后,两组患者的HADS、DHI总指数、躯体、功能及情感评分均低于治疗前,差异有统计学意义(P<0.05)。治疗30 d后,研究组HADS、DHI总指数、躯体、功能及情感评分低于对照组,差异有统计学意义(P<0.05)。结论在常规抗抑郁药物的基础上联合小脑顶核电刺激疗法可能更有利于改善慢性主观性头晕患者的头晕症状,并能有效改善患者伴发的焦虑、抑郁症状。

Objective To investigate the effect of cerebellar fastigial nucleus electrical stimulation treating chronic subjective dizziness.Methods Fifty-eight patients with chronic subjective dizziness treated in our hospital from November 2018 to December 2019 were enrolled in the study as subjects.They were divided into the study group(n=29)and the control group(n=29)according to the random number table method.Conventional antidepressant treatment was performed in the control group and the study group was given cerebellar fastigial nucleus electrical stimulation based on the above measures.And all the patients in the study were assessed by dizziness handicap inventory(DHI)and hospital anxiety and depression scale(HADS).Results There were no significant differences between the two groups in any of the assessments before treatment(P>0.05).After 30 days of treatment,the HADS score,scores of DHI total index,body,function,emotion in the two groups were lower than those before treatment,and the differences were statistically significant(P<0.05).After 30 days,the HADS score,scores of DHI total index,body,function,emotion in the study group were lower than those of control group,and the differences were statistically significant(P<0.05).Conclusion On the basis of conventional antidepressant drugs,combined with cerebellar crest nuclear stimulation therapy may be more beneficial to improving the dizziness symptoms of patients with chronic subjective dizziness,and can effectively improve the symptoms of anxiety and depression.
