糖尿病肾病“肾络伏风”病机学说及“从风论治”治法的探讨

Discussion on the pathogenesis theory of‘wind evil hides in kidney collateral’and the‘treatment from the wind governs’for diabetic nephropathy

本研究团队在继承施今墨、祝谌予、吕仁和等医家学术思想的基础上,提出糖尿病肾病“肾络伏风”病机学说,认为在肾络“微型癥瘕”形成的基础上,络风内动是糖尿病肾病的病机关键。因此强调在治疗上既要重视针对肾络“微型癥瘕”的治疗,同时也要重视针对“肾络伏风”的治疗。除此之外,由于络脉遍布全身,不同部位的络脉“微型癥瘕”形成并络风内动而导致了消渴病的不同并发症。因此,治疗上应重视辨病、辨证、辨症相结合,根据不同络脉风动的情况以及不同的并发症选择不同的风药治疗以提高临床疗效。

On the basis of the academic thoughts of thoughts of SHI Jinmo,ZHU Shenyu,LV Renhe,and other physicians,the research team proposed the pathogenesis theory of‘wind evil hides in kidney collateral’of diabetic nephropathy,and believed that on the basis of‘micro abdominal mass’is the key to pathogenesis of it.Therefore,it is emphasized that attention should be paid not only to the treatment of‘micro abdominal mass’of kidney collateral,but also to the treatment of‘wind evil hides in kidney collateral’.In addition,because the collaterals are all over the body,the development of‘micro abdominal mass’of different parts of the collaterals,and‘internal stirring of collateral wind’lead to different complications of Xiaoke disease.Therefore,the treatment should attach importance to the combination of disease differentiation,syndrome differentiation and symptom differentiation,according to the wind stirring conditions of different collaterals and different complications to choose different carminative to treat,so as to improve the clinical efficacy.
