外周置入中心静脉导管换药实践与效果评价

Practice and effect Evaluation of dressing change of Peripheral Central Venous Catheter

目的:探讨贴膜外换药方法对外周置入中心静脉导管(PICC)术后并发症的影响。方法:将2015年1月至2016年12月在本中心首次置入PICC导管患者分别采用常规换药方法或贴膜外换药方法进行局部换药,比较导管感染、皮疹皮炎与导管移位等并发症的发生率。结果:采用常规换药方法进行局部换药,发生导管感染13例占5.65%,皮疹皮炎36例占15.70%,导管移位10例占4.35%。采用贴膜外换药方法局部换药出现导管感染2例占0.86%,皮疹皮炎11例占4.74%,导管移位3例占3.94%。两组比较,差异有统计学意义(P<0.05)。结论:采用贴膜外换药维护方法局部换药,解决了普通换药消毒清理不彻底、局部皮肤损伤上的不足,减少了PICC置管术后并发症的发生,延长了导管使用时间。

Objective:To study the effect of New external dressing method on complications after Peripheral Catheterization of Central Venous Catheter(PICC).Methods:The patients who were implanted with PICC catheter for the first time from January 2015 to December 2016 were selected as the subjects,and 230 patients with PICC catheter from January 2015 to December 2015 were selected as the control group.From January 2016 to December 2016,232 cases of PICC catheter were placed in the experimental group,and the new method of local dressing change was used to compare the incidence of complications such as catheter infection,rash dermatitis and catheter displacement between the two groups.Results:There were 13 cases of catheter infection(5.65%),36 cases of rash dermatitis(15.70%)and 10 cases of catheter displacement(4.35%).There were 2 cases of catheter infection(0.86%),11 cases of rash dermatitis(4.74%)and 3 cases of catheter displacement(3.94%).There was significant difference between the two groups(P<0.05).Conclusions:The application of local dressing can reduce the incidence of local skin injury and complications after PICC catheterization,and prolong the use time of catheter.
