我院门诊老年冠心病患者抗血小板药物的应用调查与分析

Investigation and analysis of antiplatelet drugs used in elderly outpatients with coronary heart disease in our hospital

目的研究我院门诊老年冠心病患者抗血小板药物的使用情况。方法随机抽取2018年1~12月我院门诊老年冠心病患者的处方,每月100张,共计1200张进行调查分析,统计抗血小板药物的使用情况。结果铝镁匹林的使用频率为5.67%,阿司匹林的使用频率为53.67%,氯吡格雷的使用频率为23.67%,替格瑞洛的使用频率为0.67%;使用抗血小板药物的处方共计848张(占比70.67%),其中使用双联抗血小板药物的处方148张(占比12.33%),使用一种抗血小板药物的处方700张(占比58.33%);用药频度前2位是阿司匹林和氯吡格雷;不合理处方12张,占比1.00%,所有不合理的处方均为氯吡格雷与质子泵抑制剂存在不良药物相互作用。结论我院门诊老年冠心病患者抗血小板药物的使用频率较高,药物品种仍以传统的抗血小板药物(阿司匹林)为主;抗血小板药物的使用基本合理,但应重点关注氯吡格雷与质子泵抑制剂的药物相互作用。

Objective To investigate the use of antiplatelet drugs in elderly outpatients with coronary heart disease in our hospital.Methods Altogether 100 prescriptions were randomly extracted from elderly outpatients with coronary heart disease every month from January to December 2018 and a total of 1200 prescriptions were selected for investigation analysis.Then the use of antiplatelet drugs was counted and analyzed.Results The percentage of the use of Aluminum Magnesium Aspirin,Aspirin,Clopidogrel and Ticagrelor was 5.67%,53.67%,23.67%and 0.67%,respectively.Among them,antiplatelet drugs were used in 848 prescriptions(70.67%),including 148 prescriptions(12.33%)for two antiplatelet drugs used and 700 prescriptions(58.33%)for one antiplatelet drug used.The most frequently prescribed antiplatelet drugs were Aspirin and Clopidogrel.Meanwhile,12 unreasonable prescriptions(1.00%)were found in this study and all the unreasonable prescriptions were accounted for adverse drug interactions between Clopidogrel and proton pump inhibitors.Conclusion The proportion of antiplatelet drugs used in elderly outpatients with coronary heart disease in our hospital is relatively high and the traditional antiplatelet drug(Aspirin)is still the main used drug.The use of antiplatelet drugs is basically reasonable,but more attention to drug interactions between Clopidogrel and proton pump inhibitors should be paid.
