腰骶角及症状体征综合评分对L5~S1椎间盘突出患者的辅助诊断价值

The Value of Comprehensive Score of Lumbosacral Angle and Symptoms and Signs in the Diagnosis of Patients with L5-S1 Disc Herniation

目的:探讨腰骶角和症状体征综合评分对L5~S1椎间盘突出的辅助诊断价值。方法:收集本院2018年6月-2019年6月经MRI证实的仅L5~S1椎间盘突出患者110例(LDH组),以及100例MRI检查结果为阴性的受检者(正常组)的临床资料。以Grogkopff法测量两组受检者X线平片的腰骶角,并查看相应受检者腰痛、腿痛、直腿抬高试验、直腿增强试验、下肢感觉、下肢肌力以及跟腱反射后进行制表得出综合评分,比较两组腰骶角大小及综合评分。结果:正常组平均腰骶角低于LDH组平均腰骶角,差异有统计学意义(P<0.001)。LDH组综合评分明显高于正常组,差异有统计学意义(P<0.001)。结论:测量腰骶角及症状体征综合评分对L5~S1椎间盘突出诊断有参考价值。

Objective:To evaluate the value of lumbosacral angle and comprehensive score of symptoms and signs in the diagnosis of L5-S1 disc herniation.Method:Clinical data were collected from 110 patients with L5-S1disc herniation confirmed by MRI in our hospital from June 2018 to June 2019 (LDH group) and 100 subjects with negative MRI results (normal group).The lumbosacral angles of the X-ray plain films of the two groups were measured by the Grogkopff method,the comprehensive score was obtained after checking the corresponding subjects’ lumbosacral pain,leg pain,straight leg elevation test,straight leg enhancement test,lower limb sensation,lower limb muscle strength and achilles tendon reflex.Lumbosacral angle size and comprehensive score were compared between the two groups.Result:The average lumbosacral angle of the normal group was lower than that of the LDH group,the difference was statistically significant (P<0.001).The comprehensive score of LDH group was significantly higher than that of normal group,the difference was statistically significant (P<0.001).Conclusion:The measurement of lumbosacral angle and the comprehensive score of symptoms and signs is of reference value for the diagnosis of L5-S1 disc herniation.
