BI-RADS结合超声弹性成像技术对筛选可行Mammotome手术的乳腺肿块患者的价值 被引量:3

Value of BI-RADS Combined with Ultrasound Elastography in Screening Patients with Breast Masses for Feasible Mammotome Surgery

摘要 目的:探讨超声乳腺影像报告和数据系统(BI-RADS)结合超声弹性成像技术在筛选可行Mammotome手术的乳腺肿块患者的价值。方法:选择2016年6月-2017年12月于本院拟行Mammotome手术的乳腺肿块患者258例,乳腺肿块共573个。对所有肿块进行BI-RADS系统分类及超声弹性成像检查。采用病理结果作为金标准,分析BI-RADS系统分类、超声弹性成像及两者联合在筛选可行Mammotome手术的乳腺肿块患者的灵敏度、特异度、阳性预测值、阴性预测值及诊断符合率。结果:BI-RADS分类灵敏度为56.67%、特异性为95.95%、阳性预测值为43.60%、阴性预测值为97.57%及诊断符合率为93.89%。超声弹性成像技术灵敏度为60.00%、特异度为95.02%、阳性预测值为40.00%、阴性预测值为97.73%及诊断符合率为93.19%。联合检测的灵敏度为43.44%、特异度为99.45%、阳性预测值为81.25%、阴性预测值为96.95%及诊断符合率为96.51%。联合检测的特异度及诊断符合率均高于两者单独检测(P<0.05)。结论:BI-RADS结合超声弹性成像可以提高诊断乳腺肿块病变危险程度的特异性及诊断符合率,对筛选可行Mammotome手术的乳腺肿块患者的临床价值较高,可临床推广。 Objective:To investigate the value of breast imaging reporting and data system (BI-RADS) combined with ultrasonic elastography in the screening of patients with breast masses for feasible Mammotome surgery.Method:A total of 258 patients planned to have Mammotome surgery in our hospital from June 2016 to December 2017 were selected.The total number of breast masses was 573.All the masses were classified by BI-RADS system and examined by ultrasound elastography.Pathological results were used as the gold standard,the sensitivity,specificity,positive predictive value,negative predictive value and diagnostic compliance rate of BI-RADS system classification,ultrasonic elastography,and the combination of the two in screening of patients with breast masses for feasible Mammotome surgery were analyzed.Result:The sensitivity of BI-RADS classification was 56.67%,the specificity was 95.95%,the positive predictive value was 43.60%,the negative predictive value was 97.57%,and the diagnostic coincidence rate was 93.89%.The sensitivity of ultrasonic elastography was 60.00%,specificity was 95.02%,positive predictive value was 40.00%,negative predictive value was 97.73%,and diagnostic coincidence rate was 93.19%.The sensitivity of the combined detection was 43.44%,specificity was 99.45%,positive predictive value was 81.25%,negative predictive value was 96.95%,and diagnostic coincidence rate was 96.51%.The specificity and diagnostic coincidence rate of the combined detection were higher than those of the two separate detection (P<0.05).Conclusion:BI-RADS combined with ultrasound elastography score can improve the specificity and diagnostic coincidence rate of breast mass lesion risk degree,which has a high clinical value for screening breast mass patients with feasible Mammotome surgery and can be promoted clinically.
